摘 要:目的探讨饮酒与首次缺血性卒中患者颅内外血管狭窄发生率、严重程度、累及血管数量的相关性。方法连续性纳入2016年1月至2019年4月在我院就诊的首次缺血性卒中并行脑血管狭窄检查的患者384例,根据有无血管狭窄分为狭窄组(278例)和非狭窄组(106例)。结果整体患者人群中,颅内动脉狭窄较颅外动脉狭窄常见(70.9%vs.19.4%),前循环血管狭窄较后循环常见(57.6%vs.15.8%)。饮酒组患者中狭窄血管位于前后循环(32.3%vs.19.5%,P=0.008)及累及双侧血管(30.3%vs.16.3%,P<0.001)的比例显著高于非饮酒组。饮酒组患者颅内外狭窄血管数量(P=0.005)及狭窄程度总评分(P=0.015)显著高于非饮酒组。结论饮酒可能与更广泛的颅内外血管狭窄及严重程度相关,可能在一定程度上促进颅内外血管狭窄的发生发展。Objective To investigate the correlation between drinking and the incidence,severity,and number of vessels involved in intracranial and extracranial vascular stenosis in patients with first ischemic stroke.Methods Continuity included 384 cases of patients with first ischemic stroke and cerebrovascular stenosis who were admitted to our hospital for the first time from January 2016 to April 2019.They were divided into stenosis group(278 cases)and non-stenosis group(106 cases)according to the presence or absence of vascular stenosis.Results In the overall patient population,intracranial arterial stenosis was more common than extracranial arterial stenosis(70.9%vs.19.4%),and anterior circulation vessel stenosis was more common than posterior circulation(57.6%vs.15.8%).In the drinking group,the proportion of stenosed vessels in the anterior-posterior circulation(32.3%vs.19.5%,P=0.008)and the involvement of bilateral vessels(30.3%vs.16.3%,P<0.001)were significantly higher than those in the non-drinking group.The number of intracranial and extracranial stenotic vessels(P=0.005)and the total score of stenosis(P=0.015)in the drinking group were significantly higher than those in the non-drinking group.Conclusion Drinking alcohol may be related to more extensive intracranial and extracranial vascular stenosis and severity,and may promote the development of intracranial and extracranial vascular stenosis to a certain extent.
