摘 要:目的探讨ω-3多不饱和脂肪酸对结直肠癌根治术患者的影响。方法收集2017年6月~2019年1月在本院确诊后行结直肠癌根治术的患者共计100例,按照随机表法分为对照组和观察组,每组50例,对照组于术前第7天至术后第7天予以一定剂量常规肠外营养支持治疗(10%鱼油脂肪乳),观察组在对照组的治疗基础上联用ω-3多不饱和脂肪酸进行同等治疗。比较两组分别在术前第7天及术后第7天血液样本检测的炎症反应指标的水平变化(包含白细胞、中性粒细胞、C-反应蛋白、降钙素原、白介素-6)和营养指标(包含TP、ALB、TFN、PA、LYM)的变化以及手术后功能恢复情况、不良反应和并发症的发生率。结果术前第7天,两组的各项炎症反应指标和营养状态指标水平进行比较,差异无统计学意义(P>0.05);术后第7天,两组WBC、N水平较治疗前均有明显提升,且观察组WBC、N水平较对照组明显升高(P<0.05);两组CRP、PCT、IL-6水平较治疗前有明显下降,且观察组CRP、PCT、IL-6水平较对照组明显降低(P<0.05)。术后第7天,两组TP、ALB水平较治疗前明显升高,且观察组TP、ALB水平较对照组明显升高(P<0.05);两组PA、TFN、LYM水平较治疗前明显降低,且观察组PA、TFN、LYM水平较对照组明显降低(P<0.05)。治疗后观察组的肛门恢复排气时间显著短于对照组,术后不良反应和并发症的发生率均明显低于对照组(P<0.05)。结论ω-3多不饱和脂肪酸用于结直肠癌手术根治术后可以有效抑制机体炎症反应,增强免疫功能,促进肠道功能恢复,明显提高患者的营养水平,降低术后并发症的发生率,进而改善患者预后和康复,值得临床上广泛使用。Objective To investigate the effect of ω-3 polyunsaturated fatty acids on the antitumor effect of colorectal cancer patients undergoing radical operation. Methods A total of 100 patients with colorectal cancer who were diagnosed in our hospital underwent radical resection from June 2017 to January 2019 were divided into control group and observation group according to the random table method, with 50 cases in each group. The control group was given a certain dose of routine parenteral nutrition support(10% fish oil fat emulsion) from the 7 th day before operation to the 7 th day after operation. The observation group was given ω-3 polyunsaturated fatty acids on the basis of the treatment of the control group. The changes of inflammatory response indexes(including leukocytes, neutrophils, C-reactive protein,procalcitonin, interleukin-6), nutritional indexes(including TP, ALB, TFN, PA, LYM) and functional recovery, adverse reactions and complications were compared between the two groups on the 7 th day before and after operation, respectively. Results On the 7 th day before operation, there was no significant difference in inflammatory response and nutrition indexes between the two groups(P>0.05). On the 7 thday after operation, the levels of WBC, N between the two groups were significantly higher than those before the operation, the levels of WBC,N in the observation group were significantly higher than those in the control group(P<0.05). The levels of CRP, PCT, IL-6 between the two groups were significantly lower than those before the operation, the levels of CRP, PCT, IL-6 in the observation group were significantly lower than those in the control group(P<0.05). On the 7 th day after operation, the levels of TP, ALB between the two groups were significantly higher than those before the operation, the levels of TP, ALB in the observation group were significantly higher than those in the control group(P<0.05).
