自锁托槽矫治器对儿童牙颌畸形矫正效果及美观度的影响

摘　　要：目的:采用自锁托槽矫治器对儿童牙颌畸形进行矫正,观察矫正效果及对美观度的影响。方法:选取笔者医院牙颌畸形儿童90例进行研究,随机将患儿分为对照组和研究组,每组45例,对照组患儿采用传统托槽矫治器治疗,研究组则采用自锁托槽矫治器。比较两组患儿临床疗效、牙周指数、疼痛评分、美观效果及炎症因子水平。结果:治疗后,研究组有效率显著高于对照组(P<0.05);两组患儿菌斑指数(PLI)、牙龈指数(GI)及龈沟出血指数(SBI)显著降低(P<0.05),牙龈退缩指数(GR)显著升高(P<0.05),且研究组升高或降低幅度明显大于对照组(P<0.05);研究组患者治疗1d、1周后的疼痛评分显著低于对照组(P<0.05);研究组美观效果显著高于对照组(P<0.05);两组患儿肿瘤坏死因子-α(TNF-α)、血管内皮生长因子(VEGF)、肾上腺皮质素释放因子(CRF)、前列腺素E2(PGE2)的水平均显著降低(P<0.05),且研究组显著低于对照组(P<0.05)。结论:应用自锁托槽矫治器矫治儿童牙颌畸形疗效显著,可明显改善牙周指数、疼痛情况和美观效果,减少炎症和应激反应,安全舒适,值得临川推广应用。Objective To correct the dental malformation in children by self-ligation bracket appliance, and to observe the correction effects and its influence on the aesthetics. Methods Ninety children patients with dental malformation in our hospital were enrolled in the study. The children patients were randomly divided into control group and study group, with 45 cases in each group. Control group was treated with conventional bracket appliance, and study group was given self-ligation bracket appliance. The clinical efficacy, periodontal index, pain score, aesthetic effect and inflammatory factors levels were compared between the two groups. Results After treatment, the effective rate of treatment in study group was significantly higher than that in control group(P<0.05). The plaque index(PLI), gingival index(GI) and sulcus bleeding index(SBI) were significantly decreased in the two groups(P<0.05)while the gingival recession index(GR) was significantly increased(P<0.05), and the increase or decreases in study group were significantly greater than those in control group(P<0.05). The pain score in study group at 1 d and 1 week was significantly lower than that in control group(P<0.05). The aesthetic effects in study group were significantly higher than those in control group(P<0.05).The levels of tumor necrosis factor-α(TNF-α), vascular endothelial growth factor(VEGF),corticoid releasing factor(CRF) and prostaglandin E2(PGE2) were significantly decreased in the two groups(P<0.05), and the levels in study group were significantly lower than those in control group(P<0.05). Conclusion Self-ligation bracket appliance has significant efficacy in the treatment of dental malformations in children. And it can significantly improve the periodontal indexes, pain and aesthetic effects, reduce inflammation and stress response, and it is safe and comfortable and worth promoting.
