摘  要:目的:探讨卡培他滨治疗对HER-2阳性乳腺癌患者肿瘤标志物、抑癌基因表达及心肌损伤的影响。方法:根据患者治疗方式差异,将70例HER-2阳性乳腺癌患者分成对照组和观察组,每组各35例。对照组接受替吉奥治疗,观察组接受卡培他滨治疗,比较两组患者肿瘤标志物、抑癌基因表达、心肌损伤以及炎症因子水平变化。结果:治疗前,两组患者血清肿瘤标志物、炎症因子及心肌损伤因子水平比较,差异无统计学意义(P>0.05);治疗2个疗程后,两组患者血清CEA、CA153、TPS、CRP、TNF-α均较治疗前降低(P<0.05),患者血清CK-MB、cTnI水平均较治疗前升高(P<0.05)。其中,观察组患者血清CEA、CA153、TPS、CK-MB、cTnI、CRP、TNF-α水平均明显低于对照组(P<0.05);抑癌基因PTEN、ARID1A、Bax、Caspase-3、FasL的mRNA表达量均明显低于对照组(P<0.05)。结论:卡培他滨治疗HER-2阳性乳腺癌能更有效地降低患者肿瘤标志物水平、降低患者炎症症状、控制患者心肌损伤,促进细胞凋亡,有推广应用价值。Objective:To discuss the effects of capecitabine therapy on tumor markers,expression of tumor suppressor genes and myocardial injury in HER-2 positive breast cancer patients.Methods:70 cases of HER-2 positive breast cancer patients were selected.They were randomly divided into two groups according to the treatment methods,including 35 patients receiving tegio treatment in the control group,and another 35 patients receiving capecitabine treatment in the observation group.The levels of tumor markers,tumor suppressor gene expression,myocardial injury and inflammatory factors were compared between the two groups.Results:Before treatment,there were no significant differences in serum levels of tumor markers,myocardial injury factors and inflammatory factors between the two groups(P>0.05).After treatment for 2 courses,the levels of serum CEA,CA153,TPS,CRP and TNF-αwere lower than those before treatment(P<0.05).The serum levels of CK-MB and cTnI were higher than those before treatment(P<0.05).In the observation group,the serum levels of CEA,CA153,TPS,CK-MB,cTnI,CRP and TNF-αwere significantly lower than those in the control group(P<0.05).The mRNA expression of PTEN,ARID1A,Bax,Caspase-3 and FasLwas significantly lower than that of the control group(P<0.05).Conclusion:Capecitabine in the treatment of HER-2 positive breast cancer patients can more effectively decrease the level of tumor markers,reduce the symptoms of inflammation,control myocardial damage,promote cell apoptosis,and it has development application value.
