摘　　要：目的观察胸腹腔镜下食管癌根治颈部吻合术治疗高龄食管癌患者的围术期效果。方法回顾性分析36例行胸腹腔镜下食管癌根治颈部吻合术治疗的高龄(≥70岁)食管癌患者的临床资料,记录患者围术期相关指标,以及术后并发症发生情况。结果所有患者均顺利完成手术,手术时间为(255.14±40.83)min,术中出血量为(140.53±35.24)mL,清扫淋巴结数量为(16.50±3.57)枚,住院时间为(15.12±3.27)d。围手术期无死亡病例。术后发生轻度声音嘶哑6例,颈部吻合口瘘3例,肺部感染6例,心律失常15例,均经治疗后痊愈出院。结论胸腹腔镜下食管癌根治颈部吻合术治疗高龄食管癌患者围术期创伤小、出血少、肺部并发症发生率及死亡率低,安全性较高。Objective To observe the perioperative efficacy of thoracoscopic radical esophagectomy and cervical anastomosis for the treatment of elderly patients with esophageal cancer.Methods The clinical data of 36 elderly patients(aged 70 years or older)with esophageal cancer treated by thoracoscopic radical esophagectomy and cervical anastomosis were analyzed retrospectively.The perioperative relevant indices and the incidence of postoperative complications were recorded.Results All patients completed the operation successfully,with the operation duration of 255.14±40.83 minutes,the intraoperative blood loss of 140.53±35.24 mL,the number of lymph nodes dissected of 16.50±3.57,and the hospital stay of 15.12±3.27 days.No death occurred in perioperative period.Six cases of mild hoarseness,3 cases of cervical anastomotic leakage,6 cases of pulmonary infection and 15 cases of arrhythmia occurred after operation and then discharged from hospital after recovery.Conclusion Thoracoscopic radical esophagectomy and cervical anastomosis is a safer treatment for elderly patients with esophageal cancer with minor trauma,less blood loss,low incidence rate of pulmonary complications and low mortality in perioperative period.
