摘 要:目的建立一种高仿真的硬支气管模拟训练模型并评估其效果。方法将肺部CT扫描的DICOM文件导入MIMICS,三维重建支气管并导入3D打印机,打印硅胶支气管模型。将该模型与AirSim Trucorp儿童头部模型连接,建立完整的模拟气道。8名低年资耳鼻咽喉头颈外科医师在3D打印支气管模拟训练系统上进行2 h的模拟训练,记录并比较训练前后连续5次钳夹异物取出平均成功率,3次支气管镜检查异物成功取出平均时长,并比较操作前后信心指数。结果训练后,低年资医师连续5次钳夹异物取出平均成功率从12.5%±10.4%提高到57.5%±16.7%,3次支气管镜检查异物成功取出平均时长从(14.1±2.3)min减少到(7.8±1.3)min,对使用及指导学生操作支气管镜等各方面信心指数均有提升。上述所有指标均显示差异有统计学意义(P值均<0.05)。结论3D打印支气管模型具有较好的仿真度,对硬支气管镜检查异物取出术能起到有效的训练效果。Objective To setup a novel 3 D printed bronchial model and to evaluate its effectiveness on rigid bronchoscopy training.Methods The rubber-like translucent anatomically correct bronchia was made with 3 D printing technology.After 2 hours,training,the average operation success rate,time and confidence index in eight junior doctors were compared.Results The 3 D printed bronchial model was comparable to a porcine model in validity and satisfaction.After training,the average operation success rate increased from 12.5%±10.4%to 57.5%±16.7%,the average operating time decreased from(14.1±2.3)min to(7.8±1.3)min and all aspects of confidence index increased.All of the above indicators showed significant differences(P<0.05).Conclusions The 3 D printed bronchial model showed high fidelity and effectiveness,and may play an important role in the standardized residents training in rigid bronchoscopy.
