摘  要:①目的探究中性粒细胞与淋巴细胞比值(NLR)、前列腺特异性抗原密度(PSAD)在前列腺癌诊断中的价值。②方法唐山工人医院泌尿外科2015年1月~2019年1月收治前列腺特异性抗原(PSA)10~100ng/mL患者93例,年龄52~87岁,平均年龄(70.2±6.3)岁。所有患者均通过经直肠前列腺系统穿刺(10~12针)取得病理学资料;检测所有患者中性粒细胞与淋巴细胞数目,计算NLR;通过经直肠彩色超声测量前列腺体积,计算PSAD。应用受试者回归曲线(ROC曲线)判定NLR、PSAD诊断前列腺癌的临界值,并分析NLR及PSAD在PSA10~100ng/mL时诊断前列腺癌的意义。③结果本研究病理结果证实前列腺癌44例(47%),非前列腺癌49例(53%)。NLR、PSAD诊断前列腺癌均有统计学意义(P<0.05):运用ROC曲线分析NLR在PSA10~100ng/mL时诊断前列腺癌的临界值为2.36,灵敏度、特异度分别为0.727、0.734;PSAD临界值为0.475,敏感度、特异度分别为0.75、0.693。NLR、PSAD与前列腺癌危险程度均无相关性。④结论当PSA在10~100ng/mL时,NLR、PSAD均能提高前列腺癌诊断敏感度和特异度。Objective To explore the value of the ratio of neutrophils to lymphocytes(NLR) and prostate specific antigen density(PSAD) in the diagnosis of prostate cancer.Methods The patients with urology from Tangshan Gongren Hospital from January 2015 to January 2019, patients with Prostate Specific Antigen(PSA) 10~100 ng/mL 93 patients,aged 52~87 years,mean age(70.2±6.3)years old,all patients obtained pathological data through transrectal prostate system puncture(10~12 needles),and all patients with neutrophils and lymphocytes were detected.Number,calculated neutrophil to lymphocyte ratio NLR,the prostate specific antigen density PSAD was calculated by measuring the prostate volume by rectal color Doppler ultrasound.The regression curve(ROC curve) of the subjects was used to determine the critical value of NLR and PSAD for the diagnosis of prostate cancer,and the significance of NLR and PSAD in the diagnosis of prostate cancer at PSA10~100 ng/mL was analyzed.Results The pathological results in this study confirmed 44 cases(47%) of prostate cancer and 49 cases(53%) of non-prostate cancer.NLR and PSAD were diagnosed with prostate cancer(P<0.05).ROC curve was used to analyze the critical value of NLR in the diagnosis of prostate cancer when PSA10~100 ng/mL was 2.36,and the sensitivity and specificity were 0.727 and 0.734,respectively.The critical value of PSAD was 0.475,and the sensitivity and specificity were 0.75 and 0.693 respectively.NLR and PSAD were not related to the risk of prostate cancer.Conclusion When serum PSA10~100 ng/mL,NLR and PSAD can improve the sensitivity and specificity of prostate cancer diagnosis.
