摘  要:目的:探讨640层CT全脑灌注成像技术对超早期脑梗死的应用研究。方法:选择25名临床诊断为缺血性脑病的病人,所有患者的发病时间在6h以内,全部患者接受640层CT全脑灌注检查。测量灌注减低区域及对侧镜像区的面积以及脑血流量(cerebral blood flow,CBF)、脑血容量(cerebral blood volume,CBV)、平均通过时间(mean transit time,MTT)、达峰时间(time to peak,TTP)、延迟时间(delay time,DLY)等灌注参数。结果:25名患者中有16人发现与临床症状相对应的缺血灌注区,这些患者灌注减低区域的TTP与DLY延长、CBF减低,且与镜像区正常脑实质对比差异具有统计学意义。同时本课题还以为当患者灌注减低区域面积在30cm^2以上同时CBV在2 mL/100 g之下时,应警惕溶栓治疗导致的风险。结论:640层CT全脑灌注技术可以发现超早期脑梗塞的缺血区域,对临床开展对症治疗有重要的参考价值。Objective:To investigate the clinical application value of 640 slice whole brain computed tomography perfusion technique in patient with Super-early cerebral infarction.Materials and methods:25 patients with cerebral artery blood supply deficiency were examined withwhole brain CT perfusion scan and all the patients,attack time were within 6 hours.we can acquired the measurement parameters of ischemic area and mirrored region including cerebral blood flow(CBF),cerebral blood volume(CBV),mean transit time(MTT),time to peak(TTP)and delay time(DLY).Results:Ischemic perfusion areas corresponding to clinical symptoms were found in 16 patients.In these patients,TTP and DLY increased and CBF decreased in ischemic area,which was statistically significant compared with mirror area.This study suggests that we should consider the risks of thrombolytic therapy when the ischemic area is larger than 30 cm^2 and CBV is less than 2 mL/100 g.Conclusion:Ischemia area of Super-early cerebral infarction can be found with 640 slice whole brain CT perfusion technology.It has important reference value for clinical symptomatic treatment.
