摘　　要：目的机械血栓去除术中使用球囊导引导管(balloon guide catheter,BGC)能显著降低远端血管栓塞的发生率,改善临床疗效。本课题采用前瞻性对照研究方法,了解使用BGC的血栓去除术的疗效以及相关的影响因素。方法招募首次接受Solitaire支架开通治疗的急性颈内动脉和大脑中动脉闭塞患者。根据治疗方法,将采用球囊导引导管和采用普通导引导管的患者分为球囊组和对照组。比较分析被试术前状况、术中情况和术后90 d内临床结局和并发症情况。结果术中使用球囊导引导管的取栓术的取栓次数显著少于使用普通导引导管的取栓次数。球囊组被试的术后90 d mRS评分显著低于对照组。结论本研究结果初步证明了机械取栓手术使用球囊导引导管相比于使用普通的导引导管具有优势,能改善急性脑梗塞患者的长期预后情况。Objective The application of balloon guide catheter(BGC)in endovascular mechanical treatment can significantly reduce the incidence of distal vascular embolization and improve the therapeutic effect.In the present study,a prospective controlled analysis was conducted to investigate the efficacy and the influencing factors of thrombectomy by using stent retrievers in combination with BGC.Methods The patients with acute occlusion in internal carotid artery and middle cerebral artery who were first treated with the thrombectomy using Solitaire stent were recruited.The patients treated using BGC or the general guided catheter were divided into BGC group or Control group.The preoperative status,intraoperative status and clinical outcomes and the complications within 90 days after operation of the two groups were compared.Results The number passes of BGC group was significantly lower than that of Control group.The mRS score at 90 days after operation of BGC group was significantly lower than that of Control group.Conclusion The results of the present study provide preliminary evidence that the application of BGC in endovascular mechanical treatment has an advantage over conventional guided catheters,and BGC can improve the longterm clinical outcomes of acute anterior circulation occlusions.
