摘　　要：目的探讨Solitaire AB支架机械介入取栓术联合溶栓治疗急性脑梗死的临床效果。方法选取我院2016年3月至2019年2月收治的80例急性脑梗死患者,将其中40例行单纯溶栓治疗的患者作为溶栓组,40例行Solitaire AB支架机械介入取栓术联合溶栓治疗的患者作为联合组。观察2组患者的治疗效果,采用美国国立卫生研究院卒中量表(NIHSS)评价患者治疗前后不同时间点的神经功能情况,采用Rankin量表(mRS)评价患者的肢体功能及预后状况,比较2组患者不良反应发生率。结果联合组患者的治疗总有效率高于溶栓组,差异具有统计学意义(P<0.05);所有患者治疗后1 d、1周、1个月、3个月的NIHSS评分均较治疗前降低,且联合组低于溶栓组,差异具有统计学意义(P<0.05);联合组患者治疗后的mRS评分低于溶栓组,差异具有统计学意义(P<0.05);联合组患者的不良反应发生率低于溶栓组,差异具有统计学意义(P<0.05)。结论对急性脑梗死患者应用Solitaire AB支架机械介入取栓术联合溶栓治疗可提高治疗效果,改善患者的神经功能,提高预后效果,减少不良反应的发生。Objective To explore the clinical effect of mechanical interventional thrombectomy with Solitaire AB stent combined with thrombolysis in the treatment of acute cerebral infarction. Methods A total of 80 patients with acute cerebral infarction were selected in our hospital from March 2016 to February 2019. Of whom 40 patients received thrombolytic therapy alone were in the thrombolysis group,and 40 patients received mechanical interventional thrombectomy with Solitaire AB stent combined with thrombolytic therapy were in the combined group. The clinical efficacy of the two groups were observed. The neurological function of the two groups were evaluated by NIHSS before and at different time points after treatment. The limb function and prognosis of the two groups were evaluated by Rankin scale( mRS),and the incidence of adverse reactions was compared between the two groups. Results The total effective rate of the combined group was higher than that of the thrombolysis group,the difference was statistically significant( P < 0. 05). The NIHSS scores of the two groups on 1 day,1 week,1 month and 3 months after treatment were lower than those before treatment,and the combined group was lower than the thrombolysis group,the differences were statistically significant( P < 0. 05). After treatment,the score of mRS in the combined group was lower than that in the thrombolysis group,the difference was statistically significant( P < 0. 05). The incidence of adverse reactions in the combined group was lower than that in the thrombolysis group,and the difference was statistically significant( P < 0. 05). Conclusion The application of mechanical interventional thrombectomy with Solitaire AB stent combined with thrombolytic therapy in patients with acute cerebral infarction can improve the therapeutic effect,the neurological function and the prognosis,and reduce the occurrence of adverse reactions.
