不同脑脊液引流量对自发性脑室出血患者的临床应用分析

摘　　要：目的探讨不同脑脊液引流量对自发性脑室出血患者的临床应用效果。方法回顾性分析我院2014年1月至2018年10月行脑脊液外引流术的67例自发性脑室出血患者的资料,按引流量不同分为A组(37例)和B组(30例)。分析两组的神经功能恢复情况、引流时间、术后脑积水发生率及并发症发生率。结果两组患者术后2周GCS评分、术后3个月GOS评分比较差异无统计学意义(P>0.05)。B组的引流时间明显短于A组(P <0.05);B组的术后脑积水发生率低于A组(P <0.05)。两组患者的并发症发生率比较差异无统计学意义(P>0.05)。结论脑脊液外引流术治疗自发性脑室出血的效果确切,脑脊液匀速引流且脑脊液每日总量在400 m L以下是安全的,在此范围内脑脊液引流量越多,带管时间越短,1年内脑积水发生率越低,且不会影响患者神经功能恢复情况。Objective To explore the clinical application effect of different cerebrospinal fluid drainage volume in patients with spontaneous ventricular hemorrhage. Methods The data of 67 patients with ventricular hemorrhage undergoing cerebrospinal fluid drainage in our hospital from January 2014 to October 2018 were retrospectively analyzed. Patients were divided into group A and group B according to different drainage volume. The recovery of neurological function, drainage time, incidences of hydrocephalus and complications in the the two groups were analyzed. Results No statistical difference was found in the GCS score at 2 weeks after surgery and the GOS score at 3 months after surgery between the two groups(P >0.05). The drainage time of group B was significantly shorter than that of control A(P<0.05). The incidence of postoperative hydrocephalus in group B was significantly lower than that in group A(P <0.05). No statistical difference was found in the incidence of complications between the two groups(P >0.05). Conclusions Cerebrospinal fluid drainage in the treatment of spontaneous ventricular hemorrhage has exact effect. Uniform drainage speed and daily volume of different cerebrospinal fluid lower than 400 m L is safe. Within this range, the more drainage volume, the shorter drainage time, the lower incidence of hydrocephalus within 1 year, without affecting patients’ recovery of neurological function.
