摘　　要：目的观察腋窝淋巴结保留手术应用于前哨淋巴结阴性乳腺癌患者外科治疗中的临床疗效。方法回顾性收集2015-2017年于石河子大学医学院第一附属医院确诊为乳腺癌,术中行前哨淋巴结活检术,且前哨淋巴结均为阴性,故而未行腋窝淋巴结清扫手术的183例Ⅰ期、Ⅱ期浸润性乳腺癌患者的临床资料,观察其术后局部及区域复发情况、远处转移及死亡情况。结果183例患者术后皮瓣均未出现坏死、无皮下积液,患侧上肢未发生水肿。1例患者在术后41个月出现局部胸壁复发,1例在术后14个月出现腋窝淋巴结复发,1例在术后41个月出现骨、肺及胸膜转移,2例患者因其他非本疾病原因死亡。结论腋窝淋巴结保留术对乳腺癌前哨淋巴结阴性的患者是可行的,具有并发症少、创伤小、恢复快、美容效果好等优点,提高了乳腺癌患者的生活质量。Objective To observe the clinical effect of axillary lymph node reservation in surgical treatment of breast cancer patients after negative sentinel lymph node biopsy.Methods Retrospectively collecting the clinical information about 183 patients in the First Affiliated Hospital of Shihezi University School of Medicine from 2015 to 2017,who were diagnosed with stageⅠ-Ⅱbreast cancer and were performed with sentinel lymph node biopsy during the surgery,and the sentinel lymph nodes were all negative,so they didn’t accept axillary lymph node dissection.And the local and region recurrence,distant metastasis and death after the surgery were observed.Results In 183 patients,there was no necrosis,no subcutaneous fluid,and no edema in the upper extremities of the patients.Local recurrence occurred in 1 patient 41 months after surgery,axillary lymph node recurrence occurred in 1 patient 14 months after surgery,bone,lung and pleural metastasis occurred in 1 patient 41 months after surgery,and 2 patients died from other causes.Conclusion Axillary lymph node reservation is feasible for patients with negative sentinel lymph node of breast cancer.It has the advantages of less complications,less trauma,faster recovery,better cosmetic effect,and improves the quality of life of patients with breast cancer.
