摘  要:目的探讨T1期肾透明细胞癌(CCRCC)的CT强化特征参数与Fuhrman病理分级之间的相关性。方法选取2015年4月至2019年4月间北京市昌平区中西医结合医院收治的67例T1期CCRCC患者,整理并分析其CT强化图像,根据术后病理Fuhrman核分级分组,其中Ⅰ~Ⅱ级患者作为甲组,Ⅲ~Ⅳ级患者作为乙组。观察不同分级患者CT平扫、强化参数中的始增时间(AT)、达峰时间(TTP)和峰值强度(PI),并分析与Fuhrman病理分级之间的相关性。结果CT平扫和强化显示,甲组患者CT值高于乙组,差异有统计学意义(P<0.05)。Pearson相关分析结果显示,CT平扫、高强化区皮质期、高强化区髓质期、低强化区皮质期和低强化区髓质期等参数均与Fuhrman核分级呈负相关,差异均有统计学意义(均P<0.05)。结论T1期CCRCC患者的CT强化特征参数与Fuhrman病理分级存在一定相关性,对CCRCC早期诊断和治疗具有重要参考价值。Objective To investigate the association between enhancement patterns and parameters and Fuhrman pathological grading in stage T1 clear cell renal cell carcinoma(CCRCC)on CT images.Methods Sixty-seven patients with stage T1 CCRCC treated at Beijing Changping District Hospital of Integrated Traditional and Western Medicine from April 2015 to April 2019 were selected as the study subjects.Enhanced CT images of these patients were collated and analyzed.These patients were divided into two groups based on the postoperative Fuhrman pathological grading.Patients with gradeⅠ-Ⅱcancer were included in group A and patients with gradeⅢ-Ⅳcancer were included in group B.The contrast arrival time(AT),time to peak(TTP)and peak intensity(PI)were observed on CT images of different degrees of unenhanced and enhanced scan.The association between them and Fuhrman pathological grading was analyzed.Results The unenhanced and enhanced CT scan revealed that CT value was higher in group A than in group B(P<0.05).Pearson correlation analysis found that patterns of CCRCC during the cortical phase of non-enhancement and high-enhancement,the corticomedullary phase of high-enhancement,the cortical phase of low-enhancement and the corticomedullary phase of low-enhancement were negatively associated with Fuhrman nuclear grading(P<0.05).Conclusion The enhancement patterns and parameters on CT images were associated with Fuhrman nuclear grading in patients with CCRCC,which has important reference value for the early diagnosis and treatment of CCRCC.
