甲状腺部分型囊性结节良恶性的超声评价

摘　　要：目的分析甲状腺部分型囊性结节的超声声像图,寻找恶性结节的超声征象。方法回顾性分析2013年1月-2014年10月本院收治的114例甲状腺部分型囊性结节患者超声声像图表现,其中囊性乳头状癌24例,良性结节90例,总结良恶性结节的声像图特征。结果在甲状腺部分型囊性结节中,纵横比大于1和边缘毛刺征或小分叶征恶性肿瘤有显著相关性。囊性结节内部的实性成份呈偏心分布,边缘不光滑,呈低回声,伴有微钙化,提示为恶性肿瘤。结论超声对部分型囊性结节具有极高的诊断价值,能为临床诊断与治疗提供精确的依据。Objective To seek for the ultrasonic sign of malignant nodules through the analysis of the ultrasound imaging of partially cystic thyroid nodules.Methods We retrospectively analyzed the ultrasound(US)imaging of 114 cases of partially cystic thyroid nodules, and reviewed the characteristics of 24 cases of partially cystic papillary carcinomas and 90 cases of benign partially cystic nodules.Results In partially cystic thyroid nodules, aspect ratioover 1 was significantly correlated to the malignancy with margin spicule sign or small lobulation sign. The internal solid portion of the nodule presented eccentric distribution, with non-smooth margin, presented hypoecho and micro-calcification, it demonstrated in malignant nodules than benign ones.Conclusions Ultrasound has high diagnostic value for partially cystic thyroid nodules and could provide accurate basis for clinical diagnosis and treatment.
