急性缺血性脑卒中患者血清血栓调节蛋白、凝血酶激活纤溶抑制物及相关炎性因子水平的变化及其临床意义

摘　　要：目的探讨急性缺血性脑卒中(AIS)患者血清血栓调节蛋白(TM)、凝血酶激活纤溶抑制物(TAFI)及相关炎性因子水平的变化及其临床意义。方法我院100例AIS患者(实验组)和同期健康体检者(对照组),根据神经功能缺损评分将实验组分为轻度33例、中度35例和重度32例。检测全部受试者血清TM、TAFI、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平并比较分析。结果实验组血清TM、IL-6、TNF-α水平高于对照组,TAFI低于对照组(P<0.05)。实验组血清TM、IL-6、TNF-α水平比较为重度>中度>轻度,TAFI水平比较为重度<中度<轻度(P<0.05)。血清TM、IL-6、TNF-α水平与神经功能缺损程度呈正相关,TAFI水平与神经功能缺损程度呈负相关(P<0.05)。结论AIS患者血清TM、TAFI、IL-6、TNF-α水平可反映病情的发生和发展,对疾病的预后有着重要的指导价值。Objective To investigate the changes and clinical significance of serum thromboregulatory protein(TM),thrombin activated fibrinolytic inhibitor(TAFI)and related inflammatory factors in patients with acute ischemic stroke(AIS).Methods One hundred AIS patients and 100 healthy subjects of physical examination were included as experimental group and control group,respectively.The experimental group was further divided into mild(n=33),moderate(n=35)and 32 severe(n=32)subgroups according to the neurological impairment score.Serum levels of TM,TAFI,IL-6 and TNF-αof all subjects were detected and compared.Results Serum levels of TM,IL-6 and TNF-αin the experimental group were higher than those in the control group,and TAFI was lower than that in the control group(P<0.05).Serum levels of TM,IL-6 and TNF-αwere severe>moderate>mild while TAFI levels were severe<moderate<mild(P<0.05).Serum levels of TM,IL-6 and TNF-αwere positively correlated with the degree of neurological impairment,while TAFI levels were negatively correlated with the degree of neurological impairment(P<0.05).Conclusion Serum levels of TM,TAFI,IL-6 and TNF-αin AIS patients can reflect the occurrence and development of the disease.The indexes also have important guiding value for the prognosis of the disease.
