不翻瓣即刻种植修复术应用的美学效果研究

摘　　要：目的:通过比较不翻瓣与翻瓣术行即刻种植修复患者的软组织保存量、美学效果及患者满意度,评价不翻瓣即刻种植即刻修复技术的美学疗效。方法:将患者随机分成两组,观察组采用不翻瓣即刻种植即刻修复术,对照组采用常规翻瓣术,3个月后行永久负重修复。比较两组永久修复当天及修复后6、12个月的牙槽骨吸收量、红色美学指数(PES)、红-白美学指数(PES-WES)及患者满意度,并行统计学分析。结果:观察组永久修复当天的术区近、远中牙槽嵴顶的骨吸收量显著低于对照组(P<0.05),PES、PES-WES及患者满意度评分均显著高于对照组(P<0.05),而修复6、12个月后,两组数据差异无统计学意义(P>0.05)。结论:不翻瓣即刻种植修复术的软硬组织保存量、美学效果及患者满意度在短期内显著高于翻瓣术。Objective To evaluate the short-term esthetic effect of the immediate fl apless implantation and restoration technique by comparing the preservation of soft tissues,aesthetic effect and patient satisfaction of the patients with the immediate dental implants placed with a conventional flap elevation technique or a flapless technique.Methods Patients were divided into observation group and control group.Observation group was operated using a fl apless surgical technique,and the other with a mucoperiosteal fl ap elevation.The patients were fi nished loaded restoration after 3months of operation,followed up 6 and 12months after restoration and compared the objective clinical parameters including bone absorption,pink aesthetic scores(PES),pink and white esthetic score(PES/WES)and patient satisfaction index.Results The mesial and distal alveolar crest absorption of the observation group when loaded restoration was lower than the control group(P<0.05).The objective clinical parameters,including PES,PES-WES and patient satisfaction score,were signifi cantly higher than those of the control group(P<0.05).But two groups in 6 or 12 months after loaded restoration were no significant statistic differences.Conclusion Flapless surgery of anterior maxillary teeth immediate implant can better improve the effect of the preservation of the soft tissues,aesthetic effect and patient satisfaction in a short time.
