颏下岛状皮瓣治疗口腔颌面部肿瘤的预后及对美观程度的影响

摘　　要：目的:探讨颏下岛状皮瓣治疗口腔颌面部肿瘤的预后及对美观程度的影响。方法:选取2018年1月-2019年1月在本院进行口腔颌面部肿瘤治疗的76例患者为研究对象,将其随机分为研究组与对照组,每组38例。对照组采用传统的皮瓣修复术进行治疗,研究组采用颏下岛状皮瓣治疗。比较两组治疗后的皮瓣成活率、口腔功能及美观度评分以及两组患者的恢复情况。结果:研究组皮瓣成活率为94.7%,坏死率为5.3%;对照组皮瓣成活率为73.7%,坏死率为26.3%,两组皮瓣成活率及坏死率比较,差异均有统计学意义(P<0.05)。研究组进食不便、口腔软组织不适、语言障碍、美观度评分均低于对照组(P<0.05)。研究组总有效率为92.1%,显著高于对照组的71.1%,差异有统计学意义(P<0.05)。结论:对口腔颌面部肿瘤患者采用颏下岛状皮瓣治疗,能够显著提升皮瓣成活率,利于患者口腔功能的恢复,治疗效果较好,且有助于确保其美观性,利于临床应用及推广。Objective:To explore the prognosis and the effect on the aesthetics of submental island flap in the treatment of oral and maxillofacial tumors.Method:A total of 76 patients who underwent oral and maxillofacial tumor treatment in our hospital from January 2018 to January 2019 were selected as research objects,they were randomly divided into a study group and a control group,38 cases in each group.The control group was treated with traditional flap repair,the study group was treated with submental island flap.The survival rate,oral function and aesthetic score of the flap and the recovery of the two groups were compared.Result:The survival rate of the flap in the study group was 94.7%and the necrosis rate was 5.3%.The survival rate and necrosis rate of the flap in the control group were 73.7%and 26.3%respectively,there were statistically significant differences in the survival rate and necrosis rate between the two groups(P<0.05).The scores of eating inconvenience,oral soft tissue discomfort,language barrier and aesthetics in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The total effective rate of the study group was 92.1%,significantly higher than 71.1%in the control group,the difference was statistically significant(P<0.05).Conclusion:Submental island flap treatment for oral and maxillofacial tumor patients can significantly improve the survival rate of the flap,conducive to the recovery of oral function of the patients,the treatment effect is good,and help to ensure its aesthetic,conducive to clinical application and promotion.
