血清RASSF1A基因甲基化对原发性肝癌诊断价值的Meta分析

摘　　要：目的采用Meta分析评价血清Ras相关区域家族1A(Ras association domain family 1A,RASSF1A)基因甲基化在原发性肝癌诊断中的价值。方法检索PubMed、EMBASE、Cochrane、万方和知网数据库2000年1月至2019年10月有关血清RASSF1A基因甲基化对原发性肝癌诊断的文献,最终纳入文献11篇,累计试验组819例(原发性肝癌),对照组827例(包括慢性肝炎、肝硬化、良性肝脏疾病等),用Stata 12.0软件进行Meta分析。结果血清甲基化RASSF1A基因检测肝细胞性肝癌合并敏感性(SEN)=0.56(95%CI:0.36~0.74),特异性(SPE)=0.81(95%CI:0.64~0.92),阳性似然比(PLR)=3.0(95%CI:1.5~5.9),阴性似然比(NLR)=0.55(95%CI:0.36~0.83),诊断比值比(DOR)=5(95%CI:2~14),SROC曲线下面积(AUCSROC)=0.75(95%CI:0.71~0.79)。结论血清RASSF1A甲基化检测对肝癌诊断具有一定的价值。Objective To evaluate the diagnostic value of serum Ras association domain family 1 A gene(RASSF1 A) methylation in primary hepatic carcinoma using meta-analysis.MethodsThe PubMed,EMBASE,Cochrane library,Wanfang Data and CNKI were researched for literature published between January 2000 and October 2019 about the diagnosis of primary hepatic carcinoma with serum RASSF1 A methylation.Finally,11 references were accepted,including 819 patients with primary hepatic carcinoma and 827 controls(chronic hepatitis,cirrhosis and benign liver diseases).Mata-analysis was performed using Stata 12.0 software.Results The sensitivity and specificity of RASSF1 A methylation detection for diagnosing hepatocellular carcinoma were 0.56(95%CI:0.36-0.74) and 0.81(95%CI:0.64-0.92),respectively.The positive likelihood ratio,negative likelihood ratio,diagnostic odds ratio and area under the SROC curve were 3.0(95%CI:1.5-5.9),0.55(95%CI:0.36-0.83),5(95%CI:2-14) and 0.75(95%CI:0.71-0.79),respectively.ConclusionSerum methylated RASSF1 A determination has certain value in the diagnosis of primary hepatic carcinoma.
