精神分裂症患者对视空间材料的存储及执行控制

摘　　要：目的探讨精神分裂症患者视空间工作记忆的在线存储及执行控制功能。方法选取40例精神分裂症患者(研究组)及40名健康志愿者(对照组)作为研究对象,选择空间广度测验测评视空间工作记忆储存功能,选择n-back测验范式测评视空间工作记忆的执行控制功能。同时应用阳性和阴性综合征量表(PANSS)评定患者的精神症状。结果研究组空间广度测验总分及反向任务均低于对照组(P<0.01)。在n-back测验中研究组在n不同水平上正确率均低于对照组(P<0.01),而正确反应时较对照组延长(P<0.01)。药物剂量与n=1的反应时及n=2的正确率均呈负相关(P<0.05),与n=2的反应时呈正相关(P<0.05)。结论精神分裂症患者视空间工作记忆的在线存储能力在低负荷下未见缺损,而高负荷下的在线存储能力及执行功能则较健康人明显受损。Objective To explore the online storage function and executive function of visuo-spatial working memory in patients with schizophrenia.Methods 40 patients with schizophrenia in study group and 40 healthy controls in control group were selected and assessed with space span test for online storage function,n-back task for executive function,and Positive and Negative Syndrome Scale(PANSS)for psychotic symptoms.Results The total score and factor score of reverse task in space span test in study group were all significantly lower than those in control group(P<0.01).In n-back task,study group showed lower accuracy(P<0.01)and longer reaction time than control group(P<0.01).Drug dose was negatively correlated with reaction time in 1-back task and accuracy in 2-back task(P<0.05),and positively correlated with response time in 2-back task(P<0.05).Conclusion The online storage function of visuo-spatial working memory in patients with schizophrenia shows no defect under low load;while their online storage function and executive function show obvious defect under high load compared with the healthy controls.
