团队合作降低外科重症监护病房退药率对照研究

摘　　要：目的探讨团队合作降低外科重症监护病房(SICU)退药率的效果。方法医院SICU成立由1名住院总医师,1名护士长,1名药品护士和1名办公室护士组成的工作小组。工作小组通过调查收集资料寻找退药率高的原因,思考对策并分工合作加以解决。结果研究发现,患者转科、医嘱调整、患者紧急出院等是造成退药率居高不下的主要原因。实行团队合作后,SICU每月退药率由实施前的1.53%下降至实施后的1.33%,差异有统计学意义(P<0.05)。结论采取团队合作的方式可集中多方力量,避免了单方面采取措施的弊端,调动了医护共同的积极性,可显著降低退药率。Objective To investigate the effect of team work to reduce the drug withdrawal rate in surgical intensive care unit(SICU).Methods A working group consisting of a chief resident,a head nurse,a medicine nurse and an office nurse was established in SICU.The working group investigated and collected data to find out the reasons for the high rate of drug withdrawal,thought about countermeasures and worked together to solve them.Results It was found that the main reasons for the high rate of drug withdrawal were the transfer of patients,the adjustment of medical orders and the emergency discharge of patients.After the implementation of team cooperation,the monthly drug withdrawal rate of SICU decreased from 1.53%before implementation to 1.33%after implementation,the difference was statistically significant(P<0.05).Conclusion The way of team cooperation can concentrate many forces,avoid the malpractice of unilateral measures,mobilize the common enthusiasm of doctors and nurses,and significantly reduce the rate of drug withdrawal.
