基于CT泪道造影的泪道阻塞性疾病患者鼻泪管与鼻底平面夹角研究

摘　　要：目的测量并分析泪道阻塞性疾病患者的鼻泪管与鼻底平面的夹角。方法收集泪道阻塞性疾病患者269例538只眼(健眼176只眼,患眼362只眼);其中男性62例124只眼(患眼79只眼),女性207例414只眼(患眼283只眼);年龄4~87岁,平均(53.87±14.35)岁。所有患者泪道冲洗后灌注30%碘海醇注射液,并立即行泪道CT检查。测量并分析患眼与健眼之间、男女患者患眼之间、各年龄段患眼之间的骨性鼻泪管与鼻底平面夹角。结果患眼鼻泪管与鼻底平面夹角平均值(85.71±10.90)°,健眼为(89.29±16.82)°,两者间差异有统计学意义(P<0.05)。男女患眼之间鼻泪管与鼻底平面夹角值差异有统计学意义(P<0.05)。各年龄段的夹角间差异无统计学意义(P>0.05)。结论鼻泪管与鼻底平面的夹角近似垂直,患眼夹角偏向锐角,女性患眼夹角最小,探通鼻泪管时需根据鼻泪管走行方向和鼻泪管与鼻底平面的夹角进行。Objective To measure and analyze the angle between the nasolacrimal duct and the nasal floor plane in patients with obstructive diseases of the lacrimal duct.Methods All 269 cases(176 normal eyes,362 sick eyes)of obstructive diseases of lacrimal duct,including 62 cases of male(79 sick eyes)and female 207 cases(283 sick eyes),age from 4 to 87 years old,the average age of 53.87±14.35 years,were with lacrimal passages perfusion of 30%Iohexol Injection,and then immediately treated with CT check.Between normal and sick eyes,men and women sick eyes,among ages,the angle between nasolacrimal duct and nasal bottom plane were measured and analyzed.Results The mean angle between the nasolacrimal duct and the nasal plane of sick eyes was 85.71±10.90°,less then that in normal eyes(89.29±16.82)°,P<0.05.There was significant difference in the angle between the diseased eyes of male and female(P<0.05).There was no significant difference in angle of the diseased eyes among different age groups.Conclusions The angle is approximately vertical,but in sick eges,especially in female are more acute.Probing nasolacrimal duct should be according to the direction of the nasolacrimal duct and its angle with the nose plane.
