晶状体不全脱位囊袋张力环辅助超声乳化人工晶体状植入术临床疗效观察

摘　　要：目的探讨囊袋张力环(CTR)在白内障超声乳化摘除并一期人工晶状体(IOL)手术治疗晶状体不全脱位的临床效果。方法回顾分析2014年11月至2016年11月在我院行超声乳化联合囊袋张力环植入、人工晶状体一期植入手术的43例晶状体不全脱位患者的临床资料,总结临床经验。结果43例(43只眼)手术均顺利完成,35例脱位范围小于120°者术中植入普通CTR,8例脱位范围超过120°者应用囊袋拉钩稳定其位置并植入改良CTR。术后视力较术前明显提高,术后3个月最佳矫正视力≥0.5者26只眼(66.7%)。术后第1d眼压与术前、术后1周、1月差异有统计学意义(P<0.05)。术后1周眼压稳定,与术前及术后1月比较无统计学差异(P>0.05)。随访3~18个月,人工晶状体基本保持正位,无囊袋皱缩、人工晶状体或人工晶状体-张力环复合物脱位等严重并发症发生。结论晶状体不全脱位患者采用囊袋张力环辅助下超声乳化并一期人工晶状体植入术是临床治疗白内障的安全有效方法,值得推荐和广泛使用。Objective To observe the clinical effect of capsular tension ring-assisted cataract phacoemulsification and intraocular lens implantation for the treatment of subluxated cataract.Methods A retrospective analysis was performed to analyze 43 patients with lens dislocation and underwent cataract phacoemulsification combing capsular tension ring(CTR)and intraocular lens(IOL)implantation in our hospital from November 2014 to November 2016.Results A total of 43 cases(43 eyes)were successfully operated.Common CTR was applied in 35 patients whose dislocation range was less than 120°.In 8 patients whose dislocation range were more than 120°,capsular hooks were used to fix the place of capsular bag,after phacoemulsification modified CTR(MCTR)and intraocular lens were implanted.The visual acuity was significantly improved after operation.Twenty-six eyes(66.7%)had best corrected visual acuity(BCVA)≥0.5 at 3 months after operation.The postoperative intraocular pressure increased significantly on the first day,compared with preoperative,postoperative one week,one month and three months.The intraocular pressure tended to be stable in 1 week after operation,and was no significant different compared with preoperative,postoperative 1 month.The patients were followed up for 3 to 18 months.The intraocular lens remained in the positive position without capsular stenosis,the intraocular lens and IOL-CTR complex dislocation and other serious complications.Conclusions CTR-assisted phacoemulsification and intraocular lens implantation in patients with subluxated cataract is a safe and effective method for the treatment of cataract.It is worth recommending and widely used.
