摘  要:目的探讨MR神经突起方向离散度与密度成像(NODDI)对帕金森病(PD)患者的临床应用价值.方法选取41例PD患者和22例年龄和性别匹配的正常志愿者作为对照组进行MR平扫和NODDI,量化分析PD患者黑质致密部(SNPC)的变化,在组间比较SNPC的细胞内体积分数(VIC),方向分散指数(ODI)和各向同性体积分数(VISO).多变量逻辑回归分析确定PD的独立预测因子.结果PD患者SNPC中的VIC、ODI明显低于对照组(P<0.05).多变量逻辑回归分析显示,回归模型有统计学意义,F(6,62)=8.286(P<0.01),调整R2=0.414.PD患者SNPC中的VIC值的变化与PD的相关性有统计学意义(P<0.05),SNPC中的VIC值是PD的独立预测因子.在ROC曲线分析中,对侧SNPC中的VIC显示出最佳的诊断性能.结论NODDI可以反映PD患者大脑黑质结构的微观变化,其参数值ODI、VIC可以作为PD患者的评估指标.Objective To explore the elinical application of MR neurite orientation dispersion and density imaging(NODDI)in patients with Parkinson's disease(PD).Methods 41 patients with PD and 22 normal volunteers who were matched for age and sex and olected as control group,all performed plain MR and NODDI examinations.The changes of substantia nigra pars compacta(SNPC)in patients with PD was quantitatively analyzed and the intracellular volume fraction(VIC),orientation dispersion index(ODI)and isotropic volume fraction(VISO)of SNPC were compared between the two groups.Multivariate Logistic regression analysis was used to determine the independent predictors of PD.Results The levels of VIC and ODI in SNPC of PD patients were significantly lower than those of healthy controls(P<005).Multivariate Logistic regression analysis showed that the regression model was statistically significant[F(6,62)=8.286(P<0.01)],adjustment R2=0.414.There was a significant correlation between the change of VIC value in SNPC of PD patients and PD(P<0.05),and VIC value in SNPC was an independent predictive factor for PD.In the ROC analysis,the VIC in the contralateral SNPC showed the best diagnostic performance Condusion NODDI can rflct the microscopic changes of substantia nigra structure in patients with PD.The parameter value ODI and VIC can be used as evaluation indexes for PD patients.
