乳腺叶状肿瘤的乳腺钼靶X线摄影、彩色多普勒超声表现与病理对照研究

摘　　要：目的探讨乳腺叶状肿瘤的乳腺钼靶X线和彩色多普勒超声声像图,提高术前诊断率.方法回顾性分析经手术病理证实的134例乳腺叶状肿瘤患者的临床、影像资料,对比分析其乳腺钼靶及彩色多普勒超声的影像学表现及特点.结果134例乳腺叶状肿瘤中,81例为良性,37例为交界性,16例为恶性.乳腺钼靶X线表现为边界清楚、多有钙化、晕征不常见的肿块.乳腺叶状肿瘤在彩色多普勒超声上表现为囊变少见、血流丰富的肿块,当发生囊变时,肿块恶性的可能性较高.乳腺钼靶上良性叶状肿瘤内部粗条状钙化出现率较交界性及恶性者更常见.彩色多普勒超声上内部坏死恶性叶状肿瘤中更为常见.结论乳腺钼靶X线和彩色多普勒超声检查对乳腺叶状肿瘤的诊断有一定价值,但其良恶性在影像学上无明显差异.Objective To investigate mammography and color Doppler ultrasound findings of breast phyllodes tumor and improve preoperative diagnostic rate.Methods Clinical and imaging data of 134 patients with breast plyllodes tumor confirmed by operation and pathology were analyzed retrospectively.The imaging findings and characteristics of mammography and color Doppler ultrasound were compared and analyzed.Results Among the 134 patients of breast phyllodes tumor,81 were benign,37 were borderline and 16 were malignant.The findings of mammography were wel-defined masses with calcification,halo sign was unfrequent.On color Doppler ultrasound,breast phyllodes tumor was a mass with rare cystic change and rich blood flow.When cystic change occurs,the possibility of tumor malignancy was higher.Thick and strip shaped calcification was more common in benign phyllodes tumor on mammography than in borderline and malignant tumor.Necrosis was more common in malignant phyllodes tumor on color Doppler ultrasound.Conclusion Mammography and color Doppler ultrasound are valuable in the diagnosis of breast phyllodes tumor.However,there is no significant difference between the benign and malignant breast phylodes tumor in imaging findings.
