卵巢卵泡膜纤维瘤组肿瘤的CT诊断价值

摘　　要：目的分析卵巢卵泡膜纤维瘤组肿瘤(OTFG)的影像表现.方法回顾性分析28例OTFG的临床及CT资料.结果28例患者中27例为单侧,1例为双侧,共29个肿块,其中纤维瘤21个,卵泡膜-纤维瘤7个,卵泡膜细胞瘤1个.肿块大小约4.0 cm×4.0 cm×3.2 cm^14.0 cm×12.2 cm×10.7 cm,最大径平均约(9.7±2.3)cm.19个(65.5%,19/29)呈圆形或类圆形,7个(24.1%,7/29)形态不规则,3个(10.3%,3/29)呈分叶状.25个(86.2%,25/29)病变边界清楚.25个(86.2%,25/29)肿块以实性成分为主,2个(6.8%,2/29)肿块呈囊实性,2个(6.8%,2/29)肿块以钙化为主.增强扫描15个(51.7%,15/29)轻度强化,11个(37.9%,11/29)无强化,2个(6.9%,2/29)中度强化,1个(3.4%,1/29)重度强化.13例(46.4%,13/28)合并腹水.结论OTFG在CT上常表现为单发的圆形或类圆形实性肿块,边界清楚,密度可均匀或不均匀,可伴囊变、坏死及钙化,增强扫描多为无或轻度强化,可伴有腹腔积液.Objective To explore the imaging findings of ovarian thecomar fibroma groups(OTFG).Methods Clinical and CT features of 28 patients with OTFG were analyzed retrospectively.Results There were 29 masses in 28 patients,27 patients were unilateral and 1 patient was bilateral,including 21 fibromas,7 fibrosarcomas and 1 thecoma.The size of tumors ranged from 4.0 cm×4.0 cm×3.2 cm to 14.0 cm×12.2 cm×10.7 cm,the mean maximum diameter was(9.7±2.3)cm.Among them there were 19 lesions(65.5%,19/29)appeared round or oval in shape,7(24.1%,7/29)were iregular and 3(10.3%,3/29)were lobulated.25 tumors(86.2%,25/29)were well-defined.25 tumors(86.2%,25/29)were solid in composition,2 tumors(6.8%,2/29) were mixed and 2 tumors(6.8%,2/29)had calcification dominantly.All patients underwent contrast enhanced CT scan and showed slight enhancement in 15 lesions(51.7%,15/29).no enhancement in 11 lesions(37.9%,11/29),moderate enhancerment in 2 lesions(6.9%.,2/29)and dense enhancement in 1 lesion(3.4%,1/29).Ascites were present in 13 patients(46.4%,13/28).Conclusion The CT features of OTFG are unilateral,round or oval solid mass with well-defined border,homogeneous or heterogeneous density with cystic degeneration,necrosis or calcification,slight enhancement or no-enhancement。and often accompanied by ascites.
