急性缺血性脑梗死老年患者病情及近期预后与心电图变化关系

摘　　要：目的探讨急性缺血性脑梗死(CIS)老年患者病情及近期预后与心电图变化关系。方法选择2017年8月至2018年8月该院收治的120例急性缺血性脑梗死患者作为研究对象,分别在其入院时(48 h内)、发病第7天实行12导联心电图进行监测,研究病情严重程度、预后及梗死分型与心电图变化的关系。结果120例急性脑梗死患者中,有80例(66.67%)患者出现心电图异常,主要表现异常的类型为S-T段及心律失常;检测出心电图异常患者急性脑梗死病情严重程度明显高于未检出心电图异常者(P<0.05);检出者心电图异常情况≥2次的其病情严重程度显著高于48 h内检出心电图异常者及发病第7天检出异常者(P<0.05)。结论老年急性CIS患者大多存在心电图异常情况,加大对心电图变化观察对CIS评估及近期预后具有一定价值参考。【Objective】To investigate the relationship between the condition of elderly patients with acute ischemic cerebral infarction(CIS)and recent prognosis and ECG changes.【Methods】A total of 120 patients with acute ischemic cerebral infarction admitted to our hospital from August 2017 to August 2018 were selected as subjects.A 12-lead ECG was performed on the 7 th and48 th day after onset to investigate the relationship between severity,prognosis,and infarction classification and ECG changes.【Results】Among 120 patients with acute cerebral infarction,80 patients(66.67%)had abnormal electrocardiogram,and the main abnormalities were ST segment and arrhythmia.The severity of acute cerebral infarction was significantly higher in patients with abnormal electrocardiogram than in undetected patients(P<0.05).The disease severity of those with abnormal electrocardiogram detected≥2 times was significantly higher than those with abnormal electrocardiogram detected within 48 hours and the abnormality detected on the 7 th day of onset(P<0.05).【Conclusion】Most of the elderly patients with acute CIS have abnormal electrocardiogram.Increasing the observation of ECG changes has certain value for CIS assessment and short-term prognosis.
