摘 要:乙型肝炎病毒(HBV)感染是肝细胞癌(HCC)的最主要诱因,HBV相关HCC的免疫逃逸仍然不甚明了。通过HBV相关抗原的突变抑制性受、配体互作、分泌可溶性介质、募集免疫抑制性细胞、影响免疫细胞代谢、诱发免疫细胞功能耗竭等方式,HCC可逃避免疫系统的监视和清除,或颠覆免疫系统,使HCC快速生长,加速疾病进程。近年来在解析HCC免疫逃逸机制基础上,已经探讨出若干逆转免疫逃逸的免疫治疗新途径,对进一步研发精准免疫治疗有指导意义。Hepatitis B virus(HBV) infection is the most leading cause for hepatocellular carcinoma(HCC).The mechanisms of immune escape during the process of HBV-related HCC have not been well known.HCC tumor cells can escape the immune surveillance and clearance,or subvert the immune system through utilizing mutant HBV antigens,interacting with inhibitory receptor,secreting soluble molecules,recruiting immunosuppressive cells,inducing immune cell metabolic disorders and mediating immune exhaustion.As a result,HCC tumor cells grow rapidly and the disease progression accelerates.Based on the mechanisms of HCC immune escape,several new immunotherapy strategies to reverse the immune escape have been investigated recently,which provides guidance on developing precision immunotherapy of HBV-related HCC.
