摘  要:目的探讨牙周牙髓综合征患者应用盐酸米诺环素经根管途径联合牙周袋内给药的临床效果。方法选择该院口腔科2017年1月-2019年5月收治的100例牙周牙髓综合征患者作为研究对象,按照随机数字表法分组,每组50例,对照组实施传统根管治疗,观察组采用盐酸米诺环素治疗,经根管途径、牙周袋内给药,比较两组的临床疗效、牙周状况指标、炎症因子指标、咀嚼功能指标、满意度、不良反应发生率。结果①临床总有效率、总满意率观察组分别为98%、96%,均高于对照组的86%、82%(χ^2=4.891、5.005,P<0.05)。②治疗后,观察组的龈沟出血指数(1.96±0.64)分、菌斑指数(1.14±0.36)分、软垢指数(1.17±0.34)分均低于对照组(t=5.041、4.862、5.160,P<0.05)。③治疗后,观察组的CRP(5.68±1.49)mg/L、TNF-α(10.73±2.35)mg/L、IL-6(20.05±2.79)ng/L均低于对照组(t=5.382、5.539、5.915,P<0.05)。④治疗后,观察组的咀嚼效率(83.94±12.66)%较对照组更高(t=5.266,P<0.05),其咀嚼效率恢复时间(20.81±4.87)d较对照组更早(t=5.608,P<0.05)。⑤不良反应发生率观察组为2%,对照组为14%,比较差异有统计学意义(χ^2=4.891,P<0.05)。结论盐酸米诺环素经根管途径与牙周袋内联合给药,可有效改善牙周牙髓综合征患者的牙周状况和咀嚼功能,减轻炎症反应,有利于提高患者的临床疗效和满意度,还可减少不良反应,具有良好的安全性。Objective To explore the clinical effect of minocycline hydrochloride combined with periodontal pocket administration in patients with periodontal pulp syndrome. Methods A total of 100 patients with periodontal pulp syndrome treated in the department from January 2017 to May 2019 were selected as the research subjects. They were grouped according to the random number table method, 50 cases in each group, and the control group was treated with traditional root canals. The treatment group was treated with minocycline hydrochloride and administered via the root canal route and periodontal pocket. The clinical efficacy, periodontal condition index, inflammatory factor index,masticatory function index, satisfaction, and adverse reactions of the two groups were compared. rate. Results ① The total clinical effective rate and total satisfaction rate were 98% and 96% in the observation group, which were higher than 86% and 82% in the control group(χ^2=4.891, 5.005, P<0.05). ②After treatment, the gingival crevicular bleeding index(1.96±0.64)points, plaque index(1.14±0.36)points, and soft soil index(1.17±0.34)points in the observation group were lower than those in the control group(t=5.041, 4.862, 5.160, P<0.05). ③After treatment, the CRP(5.68±1.49)mg/L, TNF-α(10.73±2.35)mg/L, and IL-6(20.05±2.79)ng/L in the observation group were lower than those in the control group(t =5.382, 5.539, 5.915, P <0.05).(4)After treatment, the chewing efficiency of the observation group(83.94 ±12.66%) was higher than that of the control group(t =5.266, P <0.05), and the recovery time of chewing efficiency(20.81±4.87)d was earlier than that of the control group(t=5.608,P<0.05). ⑤The incidence of adverse reactions was 2% in the observation group and 14% in the control group, and the difference was statistically significant(χ^2=4.891,P <0.05).
