摘 要:目的探讨粪便钙卫蛋白检测在鉴别新生儿过敏性肠炎中的应用价值。方法方便选择2018年1月-2019年5月间收治的46例新生儿感过敏性肠炎患儿作为观察组研究对象;另从该院同期就诊且最终确诊为感染性肠炎的患儿随机抽取46例作为感染性肠炎组,同期就诊且排除肠道疾病的患儿中随机选取46例作为正常对照组。同时留取3组患儿的粪便标本送检,并进行粪便钙卫蛋白定量检测。结果①感染性肠炎组患儿的粪便钙卫蛋白水平高于过敏性肠炎组和正常对照组患儿,差异有统计学意义(F=9.465,P<0.001);过敏性肠炎组和正常对照组患儿的粪便钙卫蛋白水平的差异无统计学意义(t=0.927,P=0.241);②根据3组患儿粪便钙卫蛋白指标水平的差异,绘制ROC曲线计算AUC值,AUC值为0.821,最佳诊断界值参照约登指数最大值,当粪便钙卫蛋白水平取68.15μg/g时,约登指数最大,为鉴别诊断过敏性肠炎与感染性肠炎的最佳临界值,此时灵敏度为84.6%,特异度为79.5%。结论粪便钙卫蛋白是临床上较为理想的特异性肠道炎症标志物,对于鉴别诊断新生儿过敏性肠炎与感染性肠炎具有较高的应用价值,建议临床推广。Objective To explore the application value of fecal calprotectin detection in differential diagnosis of neonatal allergic enteritis. Methods 46 children with neonatal allergic enteritis admitted to the hospital from January 2018 to May2019 were convenient selected as the observation group.46 children from the same period of hospital and finally diagnosed as infectious enteritis were randomly selected.In the infectious enteritis group, 46 children who were diagnosed at the same time and excluded from intestinal diseases were randomly selected as the normal control group.The fecal specimens of the three groups of children were taken for examination and the quantitative detection of fecal calprotectin was performed.Results 1. The level of fecal calprotectin in the children with infectious enteritis was higher than that in the allergic enteritis group and the normal control group(F=9.465, P<0.001);there was no significant difference in fecal calprotectin levels between the allergic enteritis group and the normal control group(t=0.927, P=0.241). 2.According to the difference of the levels of fecal calprotectin in the three groups of children, the ROC curve was used to calculate the AUC value, the AUC value was 0.821, and the best diagnostic cutoff value was based on the maximum value of the Yoden index. When the fecal calprotectin level was 68.15 μg/g. The Yoden index is the largest, which is the best threshold for differential diagnosis of allergic enteritis and infectious enteritis. The sensitivity is 84.6% and the specificity is 79.5%. Conclusion Fecal calprotectin is a clinically ideal marker of specific intestinal inflammation. It has a high application value for the differential diagnosis of neonatal allergic enteritis and infectious enteritis. It is recommended to be clinically popularized.
