摘  要:目的分析院外猝死患者的急救效果。方法方便选取2017年1月-2018年12月该院救治的130例猝死病例进行分组分析,其中120急救车送诊的65例为观察组,急诊科直接接诊的65例为对照组。比较两组开始接受心肺复苏时间和救治成功率,并针对其相关因素进行分析。结果观察组接受心肺复苏的平均开始时间为(6.5±1.8)min,通过急诊科直接接诊开始CPR的平均时间为(12.7±6.5)min,两组比较差异有统计学意义(t=6.128,P<0.05),且观察组复苏成功率为9.23%,对照组复苏成功率为4.61%,两组比较差异有统计学意义(χ~2=5.173,P<0.05)。经过Logistic回归分析,心室停搏、CPR开始时间等是影响患者治疗后生存状况的独立危险因素,其中年龄越大、心室停搏、CPR开始时间越晚,生存率越低(P<0.05)。在院外急救开始有效的CPR时间越短,其复苏成功率越高。结论院外猝死患者救治成功率与开始心肺复苏时间呈正相关,猝死患者应尽早开始心肺复苏。采取防治结合有利于减少猝死发生率并提高抢救成功率。Objective Analysis of emergency treatment effect of sudden death. Methods Convenient select 110 cases of sudden death treated in our hospital in January 2017 to December 2018 were divided into two groups: 65 cases received directly from emergency department as control group and 65 cases sent by 120 ambulances as observation group. The time of cardiopulmonary resuscitation and the success rate of treatment were compared between the two groups, and the related factors were analyzed. Results The average start time of cardiopulmonary resuscitation in the observation group was 6.5±1.8 min, and the average time to start CPR through the emergency department was(12.7±6.5) min. The difference between the two groups was statistically significant(t=6.128, P<0.05). The survival rate of the observation group was 9.23%, and the success rate of the control group was 4.61%. The difference between the two groups was statistically significant(χ~2=5.173,P<0.05). After logistic regression analysis, ventricular arrest and CPR start time were independent risk factors affecting the patient’s survival after treatment. The older the patient, the ventricular arrest, the later the CPR start time, the lower the survival rate(P<0.05). The shorter the CPR time that is effective at the out-of-hospital emergency, the higher the recovery success rate. Conclusion The treatment success rate of patients with sudden death outside hospital is positively correlated with the time of cardiopulmonary resuscitation. The combination of prevention and treatment is beneficial to reduce the incidence of sudden death and improve the success rate of rescue.
