不明原因肝功能异常患者自身抗体联合检测在自身免疫肝病中的诊断价值

摘　　要：目的分析不明原因肝功能异常患者血清中自身抗体联合检测对自身免疫性肝病(ALD)的诊断价值。方法收取2018年3月到2019年3月研究对象为不明原因肝功能异常患者165例作为观察组,再选取165名健康体检者作为对照组,实施自身抗体联合检测,分别对抗核抗体(ANA)、抗线粒体抗体(AMA)、抗平滑肌抗体(SMA)、抗肝肾微粒体抗体(LKM)指标进行对比。结果观察组患者ANA检出率29.1%、AMA检出率6.1%、SMA检出率3.0%,高于对照组健康体检者(P<0.05)。结论自身抗体联合检测在不明原因肝功能异常患者中具有较高的临床应用价值,能对ALD进行初步的筛查和诊断,值得临床应用。Objective To analyze the diagnostic value of combined detection of autoantibodies in serum of patients with liver dysfunction for autoimmune liver disease.Methods One hundred sixty-five patients with unknown causes of liver dysfunction were selected as the observation group,and 165 healthy patients were selected as the control group.The patients were collected from March 2018 to March 2019,and the combined detection of autoantibody was conducted.antinuclear antibody(ANA),anti-mitochondrial antibody(AMA),anti-smooth muscle antibody(SMA)and anti-liver-kidney microsomal antibody(LKM)antibody indicators were compared respectively.Results ANA detection rate of patients with unknown causes of liver dysfunction in the observation group was 29.1%,AMA detection rate was6.1%,and SMA detection rate was 3.0%,which were higher than those in the control group(P<0.05).Conclusion The combined detection of autoantibody has high clinical application value in patients with unknown origin abnormal liver function,which can carry out preliminary screening and diagnosis of autoimmune liver disease,and is worthy of further clinical promotion and application.
