摘　　要：目的分析骨髓增生异常综合征(MDS)患者骨髓T细胞免疫球蛋白黏液素3(TIM-3)表达水平及其意义。方法收集75例MDS患者及20例非MDS骨髓,应用流式细胞术测定骨髓TIM-3+干细胞(CD34+CD38-Lin-TIM-3+细胞群)数量,分析MDS患者TIM-3表达情况与患者染色体核型、病情严重程度、MDS亚型、转白率及预后的关系。结果MDS患者骨髓液CD34+CD38-Lin-TIM-3+细胞比例高于非MDS(P<0.05);不同染色体核型、不同预后评分、不同MDS亚型MDS患者骨髓TIM-3表达比较差异有统计学意义(P<0.05),随核型变差、预后评分上升、亚型进展TIM-3表达水平上升(P<0.05);MDS转白患者骨髓TIM-3表达情况高于未转白组,随访死亡组骨髓TIM-3表达高于生存组(P<0.05)。结论MDS患者骨髓造血干细胞TIM-3表达率较高,且与MDS染色体核型、病情严重程度、亚型、转白率及预后密切相关,系白血病前状态重要因子,参与MDS病情进展过程。Objective To analyze the expression level and significance of bone marrow T cell immunoglobulin mucin 3(TIM-3)in patients with myelodysplastic syndrome(MDS).Methods The bone marrows of 75 patients with MDS and 20 patients with non-MDS were collected.The quantity of bone marrow TIM-3+stem cells(CD34+CD38-Lin-TIM-3+cell population)was determined by flow cytometry.The relationship between TIM-3 expression and chromosome karyotypes,disease severity,MDS subtypes,leukemic transformation rate and prognosis was analyzed in patients with MDS.Results The proportion of CD34+CD38-Lin-TIM-3+cells in bone marrow fluid of patients with MDS was higher than that of non-MDS patients(P<0.05).There were statistically significant differences in the TIM-3 expression in bone marrow of MDS patients with different chromosome karyotypes,different prognosis scores and different MDS subtypes(P<0.05).With karyotype deterioration,increase of prognosis score and subtype progression,the TIM-3 expression level was increased(P<0.05).The TIM-3 expression in bone marrow of MDS patients with leukemic transformation was higher than that of patients without leukemic transformation,and the TIM-3 expression in bone marrow in death group at follow-up was higher than that of survival group(P<0.05).Conclusion The TIM-3 expression rate is high in bone marrow hematopoietic stem cells of patients with MDS,and it is closely related to chromosome karyotypes,disease severity,subtypes,leukemic transformation rate and prognosis.And it is an important factor in pre-leukemia status and participates in the progression of MDS.
