血清HGF、CA199、CA724联合检测在胆管癌诊断及复发预测中的价值

摘　　要：目的探讨血清HGF、CA199、CA724联合检测在胆管癌诊断及复发预测中的应用价值。方法选取100例胆管癌患者及100例良性胆管疾病患者作为研究对象。检测患者血清中HGF、CA199、CA724水平,并分析HGF、CA199、CA724联合检测在胆管癌诊断及复发预测中的价值。随访1年记录胆管癌患者复发情况。结果随访1年,复发21例,未复发79例。对照组血清中HGF、CA199、CA724水平明显低于胆管癌组(P<0.05),胆管癌复发患者血清中HGF、CA199、CA724水平显著高于未复发患者(P<0.05)。联合检测血清中HGF、CA199、CA724水平在诊断胆管癌中的灵敏度、特异度及AUC均显著高于各指标单独检测(P<0.05);联合检测血清中HGF、CA199、CA724水平对预测胆管癌复发的灵敏度、特异度及AUC均显著高于各指标单独检测(P<0.05)。结论联合检测血清中HGF、CA199、CA724水平可显著提高胆管癌的诊断价值,并可有效提高预测复发的准确度,具有较高的应用价值。Objective To explore the value of combined detection of serum HGF,CA199 and CA724 in the diagnosis and recurrence prediction of cholangiocarcinoma.Methods 100 patients with cholangiocarcinoma and 100 patients with benign cholangiopathy were selected as the study objects.The levels of HGF,CA199 and CA724 in patients'blood were measured,and the value of HGF,CA199 and CA724 in the diagnosis and recurrence prediction of cholangiocarcinoma was analyzed.Patients were followed up for 1 year to record the recurrence of cholangiocarcinoma.Results 21 cases recurred and 79 cases did not recur.The serum levels of HGF,CA199 and CA724 in the control group were significantly lower than those of the cholangiocarcinoma group(P<0.05),and the serum levels of HGF,CA199 and CA724 in the recurrent cholangiocarcinoma group were significantly higher than those of the non recurrent cholangiocarcinoma group(P<0.05);the sensitivity,specificity and AUC of the combined detection of HGF,CA199 and CA724 in the diagnosis of cholangiocarcinoma were significantly higher than those of the single detection of each index(P<0.05);the combined detection of serum HGF,CA199 and CA724 in the sensitivity,specificity and AUC in predicting the recurrence of cholangiocarcinoma were significantly higher than those of each single index(P<0.05).ConclusionThe combined detection of HGF,CA199 and CA724 in serum can significantly improve the diagnostic value of cholangiocarcinoma,and can effectively improve the accuracy of predicting recurrence.
