三种术式治疗早期肺癌患者近期效果及呼吸功能恢复情况比较

摘　　要：目的比较电视辅助小切口手术(VAMT)、多操作孔电视胸腔镜手术(MP-VATS)和单操作孔电视胸腔镜手术(SP-VATS)三种术式治疗早期肺癌患者近期效果及呼吸功能恢复情况。方法收集行VATS肺叶切除术治疗的92例早期肺癌患者的临床资料进行回顾性分析,根据治疗术式分为VAMT组、MP-VATS组和SP-VATS组。比较三组患者手术一般情况,术前、术后1个月检测肺功能指标及6 min行走距离(6MWD)、Borg呼吸困难评分。结果①三组手术时间、胸管留置时间、清扫淋巴结数及住院时间比较差异无统计学意义(P>0.05);SP-VATS组和MP-VATS组切口长度、术中出血量、引流量均显著少于VAMT组(P<0.05);SP-VATS组和MP-VATS组切口长度、术中出血量、引流量比较差异无统计学意义(P>0.05)。②SP-VATS组手术前后肺功能指标变化差值及6MWD、Borg呼吸困难评分变化差值显著低于VAMT组与MP-VATS组(P<0.05)。结论3种术式中以SP-VATS治疗早期肺癌的临床价值最高。Objective To compare the short-term effects and respiratory function recovery of patients with early stage of lung cancer treated by video-assisted mini-thoracotomy(VAMT),multi-port video-assisted thoracoscopic surgery(MP-VATS),and single-port video-assisted thoracoscopic surgery(SP-VATS).Methods The clinical data of 92 patients with early stage of lung cancer who underwent VATS lobectomy were retrospectively analyzed.According to the treatment procedures,they were divided into VAMT group,MP-VATS group and SP-VATS group.The general surgical conditions were compared among the 3 groups.Pulmonary function indexes,6min walking distance(6MWD)and Borg dyspnea score were measured before operation and at 1 month after operation.Results There were no significant differences in the operative time,chest tube indwelling time,quantity of lymph node dissection and hospital stay among the three groups(P>0.05).The incision length,intraoperative blood loss and drainage volume in SP-VATS group and MP-VATS group were all significantly less than those in VAMT group(P<0.05).There were no significant differences in the incision length,intraoperative blood loss and drainage volume between SP-VATS group and MP-VATS group(P>0.05).The differences of pulmonary function indexes and the differences of 6MWD and Borg dyspnea score in SP-VATS group were significantly lower than those in VAMT group and MP-VATS group(P<0.05).Conclusion The clinical value of SP-VATS in the treatment of early stage of lung cancer is the highest in the 3 surgical procedures.
