TACE联合RFA治疗大肝癌的效果及对血清相关指标的影响

摘　　要：目的探讨射频消融术(RFA)联合经皮肝动脉栓塞化疗(TACE)治疗大肝癌的效果及对患者血清甲胎蛋白(AFP)、糖类抗原199(CA199)、谷氨酰胺转肽酶(GGT)、血管内皮细胞生长因子(VEGF)的影响。方法选取既往治疗的110例原发性大肝癌(肝癌最大直径≥5 cm)患者,根据治疗方法分为TACE组50例、联合组60例,对比2组的临床疗效、血清指标、随访生存率等指标。结果治疗后的肿瘤病灶直径组间比较,联合组显著低于TACE组(P<0.05);同时,2组肿瘤病灶直径在治疗后均较治疗前显著的减小(P<0.05)。联合组患者的缓解率(84.48%)显著高于TACE组(58.09%)(P<0.05);联合组的总有效率(98.28%)与TACE组(93.62%)比较差异无统计学意义(P>0.05)。治疗前,联合组和对照组的AFP、CA199、GGT、VEGF水平差异无统计学意义(P>0.05);治疗后,联合组AFP、CA199、GGT、VEGF水平均低于TACE组(P<0.05);治疗后,2组患者的AFP、CA199、GGT、VEGF水平较治疗前均显著的降低(P<0.05)。联合组患者1年、2年生存率分别为77.59%、41.38%,均显著高于TACE组患者的59.57%、21.28%(P<0.05)。结论RFA联合TACE治疗大肝癌具有更加显著的临床效果。Objective To study the effect of radiofrequency ablation(RFA)combined with transcatheter arterial chemoembolization(TACE)in the treatment of large hepatocellular carcinoma and the influence on protein on serum alpha fetoprotein(AFP),carbohydrate antigen 199(CA199),glutamine transpeptidase(GGT),vascular endothelial growth factor(VEGF).Methods 110 cases of primary liver cancer(the maximum diameter of the lesions was 5cm)were retrospectively analyzed,according to the treatment methods were divided into TACE group of 50 cases,60 cases of the combined group,compared clinical curative effect,the serum index and follow-up survival rate of the 2 groups.Results The tumor diameter in the combined group after treatment was significantly lower than that of TACE group(P<0.05);tumor diameter of the 2 groups were significantly decreased(P<0.05);remission rate was 84.48%,which was higher than that of TACE group,58.09%(P<0.05);there had no significant in the total efficiency between the combined group(98.28%)and the group TACE(93.62%)(P>0.05);the difference was not statistically significant before treatment,combined treatment group and control group in AFP,CA199,GGT,VEGF level had no significant difference(P>0.05);after treatment,AFP,CA199,GGT and VEGF in the combined treatment group were significantly lower than that of TACE group(P<0.05);after the treatment.AFP,CA199,GGT and VEGF levels before treatment were significantly decreased(P<0.05);1-year and 2-year survival rates in the combined treatment group were 77.59%and 41.38%,which were significantly higher the TACE group,59.57%and 21.28%(P<0.05).Conclusion RFA combined with TACE in the treatment of large liver cancer has more significant clinical effect.
