摘　　要：王沛教授治疗结直肠癌经验丰富,其在病因病机、治法治则、辨证用药等方面的见解均有独到之处。王沛认为结直肠癌以脾肾两虚或脾胃虚弱为本,湿、痰、瘀、毒为标,本虚标实,确立了"补脾肾、调虚寒,化毒浊"的总体治则。王沛擅用生药、小毒药、引经药及角药。常用高频药物包括生半夏、半枝莲、生黄芪、九香虫、补骨脂、生薏苡仁、女贞子、猪苓、茯苓、柴胡、香附、丹参、当归、夏枯草、山药、肉豆蔻等,配伍马齿苋、苦参、红藤三药,引诸药入大肠经。临证以辨寒热为纲,注重肠腑的通畅,辨证精准,处方精炼,每份处方平均药味为14.69味。临证时谨守病机,随证加减,在提高患者生活质量,改善临床症状,延长生存期等方面均取得较好的疗效,值得借鉴。Prof.WANG Pei has accumulated a rich experience in the treatment of colorectal cancer,and he has unique understanding in etiology and pathogenesis,principle of treatment,prescription based on syndrome differentiation and so on.Prof.WANG considers that colorectal cancer’s pathogenesis is deficiency of spleen and kidney or deficiency of spleen and stomach as the root cause,phlegm,dampness,blood stasis and toxin as the manifestations,deficiency in origin and excess in superficiality.He establishes the general treatment principle of"tonifying spleen and kidney,regulating deficiency-cold,and removing toxin and turbid".He is good at using crude drugs,toxin drugs,guiding and triangular drugs.The commonly-used herbs include Banxia(raw Pinelliae Rhizoma),Banzhilian(Scutellariae Barbatae Herba),Huangqi(raw Astragali Radix),Jiuxiangchong(Aspongopus,Psoraleae Fructus),Yiyiren(raw Coicis Semen),Nuzhenzi(Ligustri Lucidi Fructus),Zhuling(Polyporus),Fuling(Poria),Chaihu(Bupleuri Radix),Xiangfu(Cyperi Rhizoma),Danshen(Salviae Miltiorrhizae Radix et Rhizoma),Danggui(Angelicae Sinensis Radix),Xiakucao(Prunellae Spica),Shanyao(Dioscoreae Rhizoma),Roudoukou(Myristicae Semen)and so on.Machijian(Portulacae Herba),Kushen(Sophorae Flavescentis Radix)and Hongteng(Sargentodoxae Caulis)guide drugs to the large intestinal meridian.Taking the differentiation of cold and heat as the outline,focus on purging Fu-organ,the syndrome differentiation is accurate and the prescription is refined.Each formula contains an average of 14.69 herbs.The treatment principle pays attention to the pathogenesis,regulate the medication with syndrome.The prescriptions apparently enhance the quality of life,relieve the patients’symptoms and prolong the survival span.WANG Pei’s experience deserves to be introduced in clinical practice.
