130名人工耳蜗术后听障儿童家长亲职压力影响因素相关性研究

摘　　要：目的探讨社会及家庭环境中人工耳蜗术后儿童家长亲职压力影响因素的相关性分析。方法采用一般情况调查表、亲职压力量表、儿童行为量表(rutter量表)、儿童生存质量量表、应对方式问卷、社会支持评定量表对甘肃地区130名人工耳蜗术后听障儿童及家长进行调查和评估。结果人工耳蜗术后。听障儿童父母平均压力为103.45分,亲职压力总分与rutter总分(r=0.499)、儿童违纪行为(A行为)(r=0.420)、儿童神经症行为(N行为)(r=0.327)、消极应对(r=0.401)呈极显著正相关(P<0.01);与父母受教育程度(父r=-0.236,母r=-0.237)、家庭月收入(r=-0.325)、儿童生活质量(r=-0.435)、社会支持(r=-0.288)、对支持的利用度(r=-0.228)呈极显著负相关(P<0.01);与主观支持(r=-0.213)、客观支持(r=-0.217)呈显著负相关(P<0.05)。逐步回归分析后可知,亲职压力受rutter总分、消极应对、母亲受教育程度、儿童生活质量及社会支持度的影响,rutter总分与消极应对方式、亲职压力呈显著正相关;母亲受教育程度与社会支持度、亲职压力呈显著负相关。结论人工耳蜗术后听障儿童家长亲职压力处于非常高的水平。儿童行为是影响家长压力的最重要因素,而父母自身的受教育程度、社会支持、经济水平及应对方式也对亲职压力有着重要影响。Objective To explore the influence of social and family environment on the stress of parents after cochlear implantation.Methods 130 children and their parents after cochlear implantation were assessed with child behavior scale,parenting stress scale,social support scale,coping style scale and general questionnaire.Results The average stress score of the parents after cochlear implantation was 103.45.The total score of parenting stress was positively correlated with Rutter Score r=0.499,children’s indiscipline(a behavior)r=0.420,children’s neurotic behavior(N behavior)r=0.327,and negative coping R=0.401 There were significant negative correlations between parental education level and father(r=-0.236),mother(r=-0.237),monthly family income(r=-0.325),quality of life of children(r=-0.435),social support(r=0.288),and utilization of support(r=-0.228)(P<0.01)There was a negative correlation with subjective support(r=-0.213)and objective support(r=-0.217)(P<0.05).After stepwise regression analysis,parenting stress was influenced by Rutter’s total score,negative coping style,mother’s education level,children’s quality of life and social support Rutter’s total score and negative coping style were positively correlated with dependent variable parenting stress,while Rutter’s total score,and negative coping style were positively correlated with dependent variable parenting stress.Conclusion Parenting stress was very high in this study.Children’s behavior was the most important factor that effected parents’stress,and parents’education,social support,economic level and coping style also had an important influence on parental stress.
