抽动障碍患者下颌骨骨折内固定断裂1例回顾分析

摘　　要：目的通过1例典型病例,分析内固定断裂原因与抽动障碍之间的关系,提高该类疾病的临床治疗效果。方法报道1例抽动障碍患者内固定术后断裂病例,结合文献复习,总结手术注意事项。结果该患者常规使用小型接骨板进行骨折切开复位内固定后出现内固定物断裂,二次手术采用下颌骨重建板进行右侧下颌骨骨折内固定,术后随访,右侧下颌骨骨折愈合良好。结论临床面对肌力或肌张力异常、不协调、亢进等情况,合并下颌骨骨折时,应充分考虑内固定应力作用,必要时使用下颌骨重建板进行坚强内固定,使骨折在骨折端稳定的条件下进行骨愈合。Objective To analyze the relationship between the cause of internal fixation breakage and tic disorder through a typical case,so as to improve the clinical treatment effect of this kind of disease.Methods A case of breakage after internal fixation in a patient with tic disorder was reported,and literature review was conducted to summarize the notes of operation.Results The patient had breakage of internal fixation after open reduction and internal fixation with small bone plate.The second operation was performed with mandible reconstruction plate for internal fixation of right mandible fracture.The right mandible fracture healed well after follow-up.Conclusion The stress of internal fixation should be taken into full consideration when patients were combined with mandibular fracture in the face of abnormal muscle strength or muscle tension,uncoordinated and hyperactivity,and the mandibular reconstruction plate should be used for strong internal fixation when necessary so that the fracture can heal under the condition of stable fracture end.
