奥沙利铂为主的两种化疗方案治疗晚期结直肠癌患者的效果比较

摘　　要：目的比较奥沙利铂为主的两种化疗方案治疗晚期结直肠癌(CRC)患者的效果。方法选择在我院进行晚期CRC治疗的116例患者,依照患者住院时间将其分为对照组[58例,FOLFOXIRI方案(奥沙利铂+伊立替康+氟尿嘧啶)]和研究组[58例,SOX方案(奥沙利铂+替吉奥)]。比较两组的治疗效果、不良反应发生情况及生存情况。结果治疗4个疗程后,两组的DCR比较,差异无统计学意义(P>0.05)。截止随访结束,两组的生存率比较,差异无统计学意义(P>0.05)。对照组的白细胞减少、血小板减少、腹泻的Ⅱ~Ⅲ级不良反应发生率明显高于研究组(P<0.05)。研究组治疗4个疗程结束时和随访时的生活质量评分均明显高于对照组(P<0.05)。结论SOX方案和FOLFOXIRI方案治疗晚期CRC的效果相当,但SOX方案治疗过程中,患者白细胞减少、血小板减少、腹泻的耐受性较好,且生活质量较高,在临床可优先考虑。Objective To compare the efficacy of two chemotherapy regimens based on oxaliplatin in the treatment of patients with advanced colorectal cancer(CRC).Methods A total of 116 patients with advanced CRC treated in our hospital were selected and divided into control group[58 cases,FOLFOXIRI scheme(oxaliplatin+irinotecan+fluorouracil)]and study group[58 cases,SOX scheme(oxaliplatin+tegafur,gimeracil and oteracil potassium)]according to the time of inhospital.The therapeutic effect,adverse reactions and survival of the two groups were compared.Results After 4 courses of treatment,there was no significant difference in DCR between the two groups(P>0.05).The end of follow-up,there was no significant difference in survival rate between the two groups(P>0.05).The incidences of gradeⅡ-Ⅲadverse reactions of leukopenia,thrombocytopenia and diarrhea in the control group were significantly higher than those in the study group(P<0.05).The quality of life scores in the study group at the end of the 4 courses of treatment and at follow-up were significantly higher than those in the control group(P<0.05).Conclusion SOX scheme and FOLFOXIRI scheme in the treatment of advanced CRC have a same effect,but during the treatment of SOX scheme,the patients have better tolerated with leukopenia,thrombocytopenia,diarrhea and high quality of life,which can be given priority considered in the clinic.
