不同垂直骨面型骨性Ⅰ类患者颞下颌关节位置与形态的差异性研究

摘　　要：目的:探讨不同垂直骨面型骨性Ⅰ类错畸形患者颞下颌关节位置与形态的差异。方法:筛选骨性Ⅰ类错畸形成人患者108例,分为低角组(37例)、均角组(39例)、高角组(32例)。对各组患者用CBCT通过Dolphin软件进行三维重建颞下颌关节影像,对影像进行测量和统计学分析。结果:骨性Ⅰ类错畸形患者双侧颞下颌关节位置与形态测量值之间无统计学差异(P>0.05)。颞下颌关节半径值、关节上间隙、髁突长度、髁突短轴、髁突长轴、关节窝长度、关节窝高度在三组间有统计学差异(P<0.05)。结论:骨性Ⅰ类错畸形患者左右侧颞下颌关节位置与形态基本对称,不同垂直骨面型各组颞下颌关节位置与形态存在差异。Objective:To investigate the differences of the position and morphology of the temporomandibular joint(TMJ)among the patients with different vertical skeletal features of skeletal class I malocclusion.Methods:108 patients with skeletal class I malocclusion were screened by lateral cephalogram and were divided into low angle group(n=37),average angle group(n=39)and high angle group(n=32).The TMJs were reconstructed in three dimensions by CBCT with Dolphin software,and the reconstructed 3D images of the TMJs were measured and statistically analyzed.Results:There was no statistical difference of the position and morphological measurements between bilateral temporomandibular joints in patients with skeletal class I malocclusion(P>0.05).There was a significant difference of radius of condyle,superior joint space,length of condyle,long axis and minor axis of condyle,length and height of articular fossa among the 3 groups(P<0.05).Conclusion:The location and morphology of the bilateral temporomandibular joints in patients with skeletal class I malocclusion were basically symmetrical,and the shape and position of the temporomandibular joints with different vertical bone type were different among the 3 gorups.
