摘 要:目的探讨三维数字减影血管造影(3D-DSA)与三维CT血管造影(3D-CTA)两种检查方式对颅内动脉瘤破裂出血患者早期诊断的应用价值。方法择取2019-01—2019-12经手术确诊为颅内动脉瘤破裂出血患者35例作为研究对象,所有患者手术前分别进行3D-DSA和3D-CTA检查,以手术确诊结果为金标准,对两种检查方式①动脉瘤检出率;②呈现瘤颈与载瘤动脉关系的能力展开比较。结果手术结果显示,35例患者共发现38个动脉瘤,其中32个单发动脉瘤,3例为双发动脉瘤,3D-DSA全部检出,检出率100%,3D-CTA检出36个,检出率94.74%,漏检2个,漏检的2个动脉瘤长颈均<3.0 mm,两种检查方式动脉瘤检出率差异无统计学意义(P>0.05)。在呈现瘤颈与载瘤动脉关系的能力上,3D-DSA较3D-CTA更理想,差异具有统计学意义(P<0.05)。结论3D-CTA与3D-DSA均可对颅内动脉瘤破裂出血的早期诊断提供可靠的依据,但3D-DSA成像效果更优,在临床上应用价值更高。Objective To explore the value of three-dimensional digital subtraction angiography(3D-DSA)and three-dimensional CT angiography(3D-CTA)in the early diagnosis of patients with ruptured intracranial aneurysm.Methods 35 patients diagnosed with intracranial aneurysm rupture and bleeding were selected as the object of this study,and the treatment time was from January 2019 to December 2019.All patients were examined by 3D-DSA and 3D-CTA before surgery.Taking the diagnosis result of surgery as the gold standard,we compared the detection rate of aneurysm and the ability to show the relationship between tumor neck and tumor-bearing artery.Results The surgical results showed that 35 patients had 38 aneurysms,including 32 single aneurysms,and 3 patients had double aneurysms.3D-DSA was all detected and so the detection rate was 100%,36 cases were detected by 3D-CTA and so the detection rate was 94.74%.The long necks of the two aneurysms that were missed were less than 3.0mm.There was no statistically significant difference in aneurysm detection rates between the two methods(P>0.05).In terms of the ability to show the relationship between the tumor neck and the tumor-bearing artery,3D-DSA is more ideal than 3D-CTA,and the difference is statistically significant(P<0.05).Conclusion Both 3D-CTA and 3D-DSA can provide a reliable basis for the early diagnosis of intracranial aneurysm rupture and bleeding,but 3D-DSA imaging is more effective and has higher clinical value.
