摘 要:目的探究游泳运动对木瓜蛋白酶诱导大鼠膝关节骨性关节炎的预防作用。方法将20只正常SD大鼠随机分为对照组和游泳组,所有大鼠膝关节腔内注射木瓜蛋白酶,游泳组大鼠注射木瓜蛋白酶后进行为期6周的游泳干预,2组均于6周后同时取材,进行关节表面肉眼大体评分及切片Mankins评分,免疫组化测定软骨中基质金属蛋白酶-13(MMP-13)表达情况,ELISA法测定关节液中白细胞介素-1β(IL-1β)水平。结果游泳组关节肉眼评分和Mankins评分分别为(0.92±0.15)分和(4.26±3.64)分,对照组分别为(1.39±0.54)分和(9.76±1.03)分,游泳组关节肉眼评分和Mankins评分均明显低于对照组(P均<0.05);游泳组软骨中MMP-13阳性面积百分比和关节液中IL-1β水平分别为(0.09±0.10)%和13.88±6.01,对照组分别为(2.67±0.39)%和38.61±0.87,游泳组软骨中MMP-13阳性面积百分比和关节液中IL-1β水平均明显低于对照组(P均<0.05)。结论游泳运动可在大鼠膝关节骨性关节炎模型形成阶段,通过抑制软骨中MMP-13、IL-1β的分泌来减少关节软骨的破坏和退化,预防膝关节骨性关节炎的发生。Objective It is to explore the preventive effect of swimming exercise on papain-induced osteoarthritis of knee joints in rats.Methods Twenty normal SD rats were randomly divided into a control group and a swimming group.All rats were injected with papain in the knee joint cavity.The rats in the swimming group were given 6 weeks of swimming intervention after injection of papain.The experimental samples were taken at the same time in both groups after 6 weeks to evaluate the gross surface score of the joint surface and the Mankins score of the slice.The expression level of matrix metalloproteinase-13(MMP-13)in cartilage was determined by immunohistochemistry.The level of interleukin-1β(IL-1β)in joint fluid was measured by ELISA.Results The macroscopic score and Mankins score of joints in the swimming group were(0.92±0.15)and(4.26±3.64)points respectively,those in the control group were(1.39±0.54)and(9.76±1.03)points respectively,and the joint macroscopic score and Mankins score were in the swimming group were significantly lower than those of the control group(all P<0.05);the percentage of MMP-13 positive area in cartilage and IL-1βlevel in joint fluid in the swimming group were(0.09±0.10)%and 13.88±6.01,those in the control group were(2.67±0.39)%and 38.61±0.87 respectively.The percentage of MMP-13 positive area in cartilage and IL-1βlevel in joint fluid of swimming group were significantly lower than those of control group(all P<0.05).Conclusion Swimming exercise can reduce the destruction and degradation of articular cartilage by inhibiting the secretion of MMP-13 and IL-1βin cartilage during the formation stage of rat knee osteoarthritis model,thus to prevent the occurrence of knee osteoarthritis.
