基于《脾胃论》探讨脾胃功能失调与血脂异常的关系及论治

摘　　要：我国血脂异常的发生率逐年增高,血脂异常作为心脑血管疾病的主要危险因素,血脂水平的调控早已成为临床医生治疗的主要靶点。现代研究发现,无论是脾脏的切除或是肠道菌群的失调均会导致血脂异常的发生。李东垣认为,脾胃受损是百病之源,脾胃受损,脾胃阳气推动无力,中焦气机升降失调,无法推动固摄血液、水液的正常运行,水谷精微转化失调,加上饮食、劳倦、风寒的侵袭,最终形成湿、痰、瘀等病理产物,导致痰瘀互结,凝于血脉,形成脂质。研究基于李东垣的《脾胃论》,探讨脾胃功能与血脂异常形成的关系,从脾胃胜衰论、饮食劳倦所伤为热中论、阴阳升降论三个方面对血脂异常进行论治,以及探讨李杲如何通过益气升阳除湿法、补脾胃泻阴火升阳法、调饮食适寒温法调理脾胃功能,为临床医生从脾胃调治血脂异提供新思路。The incidence of dyslipidemia in China has been increasing year by year. Dyslipidemia is the main risk factor for cardiovascular and cerebrovascular diseases. The regulation of blood lipid levels has been the main target for clinicians.Modern studies have found that the removal of the spleen or the imbalance of gut flora can lead to dyslipidemia. LI Dongyuan believes that damage to the spleen and stomach is the source of all diseases. The damaged spleen and stomach and impaired lifting of the Qi of Middle Jiao,can make spleen unable to promote the normal operation of blood and water,and imbalance in the subtle conversion of water and food. The invasion of tiredness and wind chills eventually result in pathological products such as dampness,phlegm,and stasis,which cause phlegm and blood stasis in the blood veins,and form lipids. Dyslipidemia is a major risk factor for cardiovascular diseases. Based on LI dongyuan’s Treatise on Spleen and Stomach,which discusses the relationship between spleen and stomach functions and the formation of dyslipidemia,and discusses the treatment of dyslipidemia from three aspects. The method of regulating the function of spleen and stomach by LI Gao was also discussed,which provided a new idea for clinical doctors to treat dyslipidemia from spleen and stomach.
