可调整巩膜瓣缝线小梁切除术与常规小梁切除术治疗青光眼的Meta分析

摘　　要：目的:比较可调整巩膜瓣缝线小梁切除术与常规小梁切除术两种手术方式治疗青光眼的安全性和有效性。方法:通过系统检索PubMed、维普数据库、万方数据库、中国知网数据库,采用统一的纳入和排除标准,用Review Manager 5.0软件进行Meta分析。结果:共纳入7个RCT,其中4个RCT的结果显示术后早期(术后第一周)两种手术方式的浅前房发生率差异有统计学意义,OR值和95%CI分别为0.10、[0.03,0.31]。3个RCT的结果显示术后早期两种手术方式控制眼压的有效性差异不存在统计学意义,OR值和95%CI分别为0.56、[0.26,1.23]。4个RCT的结果显示术后早期两种手术方式对控制视力下降差异有统计学意义,OR值和95%CI分别为0.32、[0.16,0.64]。结论:可调整巩膜瓣缝线小梁切除术与常规小梁切除术两种手术方式治疗青光眼在术后早期控制眼压的有效性方面无明显差异,在术后浅前房发生率以及控制视力下降方面,可调整巩膜瓣缝线小梁切除术优于常规小梁切除术。Objective To compare the safety and effectiveness of adjustable scleral flap suture trabeculectomy and conventional trabeculectomy for glaucoma.Methods Through the systematic search of PubMed,VIP,Wanfang and China HowNet databases,the unified inclusion and exclusion criteria were adopted,and the Review Manager 5.0 software was used for Meta analysis.Results There were 7 RCTs included in the study.The results of 4 RCTs showed that there was a significant difference in the incidence of shallow anterior chamber between the two methods during the early postoperative period(the first week).OR and 95%CI were 0.10,[0.03,0.31],respectively.The results of three RCTs showed that there was no significant difference in the effectiveness of the two early postoperative methods for intraocular pressure control.OR and 95%CI were 0.56,[0.26,1.23],respectively.The results of four RCTs showed that there was a significant difference between the two methods in the early postoperative visual acuity control.OR and 95%CI were 0.32,[0.16,0.64],respectively.Conclusion There was no significant difference in the efficacy of intraocular pressure control in the early postoperative period between trabeculectomy with adjustable scleral flap suture and conventional trabeculectomy in the treatment of glaucoma.In terms of the incidence of shallow anterior chamber and the decrease of vision control,the adjustable scleral flap suture trabeculectomy was superior to conventional trabeculectomy.
