摘 要:目的对比3D CUBE T1压脂与非压脂序列在大脑中动脉(MCA)狭窄患者高分辨率血管壁成像中的效果。方法选择105例MCA狭窄患者,均行MCA MRI检查,用3D CUBE技术分别进行T1压脂及非压脂序列扫描。计算并比较2种扫描序列各感兴趣区域的图像信号强度、信噪比、对比信噪比及噪声,2种序列测量的血管断面、管腔面积及斑块信号差异。由2名医师分2次对2种序列图像质量进行评分,分析其一致性。结果3D CUBE T1非压脂序列各感兴趣区的信号程度、信噪比、对比信噪比均高于T1压脂序列,而图像噪声低于T1压脂序列,比较差异有统计学意义(P均<0.05)。2种序列测量的血管断面及管腔面积比较差异无统计学意义(P均>0.05),测量的斑块面积比较差异有统计学意义(P<0.05)。3D CUBE T1非压脂及压脂序列均可清晰显示MCA斑块,但信号表现有差异。有12例患者在2种序列上斑块信号发生差异,在3D CUBE T1非压脂序列呈高或稍高信号,而在3D CUBE T1压脂序列呈等信号。2种序列管腔显示情况评分比较差异无统计学意义(P>0.05)。与3D CUBE T1压脂序列比较,3D CUBE T1非压脂序列伪影及噪声、图像对比度、血管显示情况、血管壁和周围组织对比情况、斑块显示情况、斑块和血管壁对比情况评分高(P均<0.05)。2名医师间第1、2次对图像的评分有良好的一致性,2名医师各自第1次与第2次对图像的评分有良好的一致性。结论MCA狭窄患者高分辨率血管壁成像时3D CUBE T1非压脂序列比压脂序列噪声低、信噪比及组织分辨率高,对血管壁及斑块的显示更具优势。Objective To compare the image effects between 3D CUBE T1-weighted sequence images with and without fat suppression in high resolution intracranial magnetic resonance imaging.Methods By using GE3.0T MRI and 3D CUBE technology,105 patients with middle cerebral artery stenosis were scanned with 3D CUBE T1-weighted sequence images with and without fat suppression,respectively.The image signal strength,signal-to-noise ratio and contrast signal-to-noise ratio of each region of interest of two different scan sequences were calculated and compared,and images of different sequences,the plaque,and lumen area were measured.The image quality of these two sequences was scored by 2 physicians for twice and their consistency was analyzed.Results The signal intensity,signal-to-noise ratio and contrast signal-to-noise ratio of each region of interest of the 3D CUBE T1 without fat suppression sequence were all higher than those of the 3D CUBE T1 with fat suppression sequence,while the noise level was lower than that of the 3D CUBE T1 fat suppression sequence,with statistically significant differences(all P<0.05).There was no statistically significant difference in the vascular cross section and lumen area measured by the two physicians(both P>0.05),but there was a statistically significant difference in the plaque area measured(P<0.05).Both 3D CUBE T1 with and without fat suppression sequences could clearly display MCA plaques,but the signal performance was different.There were 12 patients with different plaque signals on the two sequences,showing high or slightly high signals in the 3D CUBE T1 without fat suppression sequence,and showing equal signals in the 3D CUBE T1 with fat suppression.There was no statistically significant difference in lumen display scores between the two sequences(P>0.05).Compared with 3D CUBE T1 with fat suppression sequence,3D CUBE T1 without fat suppression sequence had higher scores in artifact,noise,image contrast,vascular display,vascular wall and surrounding tissue comparison,plaque display,and pla
