摘  要:目的探讨银杏二萜内酯葡胺注射液联合单唾液酸四己糖神经节苷脂(GM1)对急性脑梗死(ACI)患者神经功能的影响。方法选取2017-01—2019-03襄阳市第一人民医院神经科收治的103例ACI患者,按随机数字表法分为2组,分别为单用组51例和联用组52例。2组均给予西医常规治疗;单用组给予GM1静滴,80 mg/次,1次/d;联用组联用GM1和银杏二萜内酯葡胺注射液,25 mg/次,1次/d;2组治疗时间均为14 d。观察2组患者治疗前后格拉斯哥昏迷评分(GCS)、美国国立卫生研究院卒中量表(NIHSS)评分、血液流变学指标、Barthel指数的变化。结果与治疗前比较,治疗后联用组NIHSS评分、血液流变学指标均显著降低,GCS评分、Barthel指数均显著升高(均P<0.05)。治疗后联用组NIHSS评分、血液流变学指标显著低于单用组(均P<0.05),GCS评分、Barthel指数均显著高于单用组(均P<0.05)。结论银杏二萜内酯葡胺和GM1联合应用于治疗急性期脑梗死,能够降低血黏度,有效阻止血小板的聚集,减轻细胞的炎症反应和缺血性脑组织损伤,能够促使神经营养相关因子的分泌,抑制氧化应激反应,从而改善患者缺损的神经功能,对修复受损神经功能有重要的临床意义。Objective To observe the effect of Ginkgo Biloba Diterpenoid Lactone Glucamide injection combined with monosialic acid tetrahexose ganglioside(GM1)on neurological function in patients with acute cerebral infarction(ACI).Methods A total of 103 cases of ACI patients admitted to the Department of Neurology of Xiangyang First People's Hospital Affiliated to Hubei University of Medicine from January 2017 to March 2019 were selected.According to the random number table method,they were divided into two groupsthe single group(51 cases)and the combined group(52 cases).Both groups were given conventional treatment of western medicine.One group was given GM1 intravenous drip(iv),80mg each time,once a day.The other group was given GM1(the same as above)and Ginkgo Biloba Diterpene Lactone Glucomide injection once a day for 25 mg each time(the combined group).Both groups received treatment for 14 days.The changes of Glasgow coma score(GCS),National Institutes of Health stroke scale(NIHSS)score,Barthel index and hemorheology indexes were observed before and after treatment.Results Compared with the pre-treatment group,the NIHSS score,hemorheology indexes,GCS score and Barthel index in the post-treatment group were significantly decreased,while the GCS score and Barthel index were significantly increased(all P<0.05).After treatment,the NIHSS score and hemorheology indexes of the combined group were significantly lower than those of the single group(all P<0.05),GCS score and Barthel index were significantly higher than those of the single use group(all P<0.05).Conclusion Ginkgo Diterpene Lactone Meglumine injection and GM1 combination therapy applied in acute cerebral infarction,and can effectively prevent platelet aggregation,reduce blood viscosity,reduce the cell inflammation and ischemic brain damage,promote the secretion of neurotrophic factors related,inhibit oxidative stress reaction,improve the patient's neurologic,to repair the damaged neural function has important clinical significance.
