摘 要:目的分析血管超声影像学特征与缺血性脑卒中静脉溶栓(IVT)治疗效果的相关性。方法IVT时对入组患者进行首次TCD检查,IVT 24 h内进行血管超声检查,探讨血管超声影像表现与静脉溶栓治疗后“显著改善”是否相关。结果在侧支循环、溃疡性斑块、房颤、右向左分流几个因素上,显著改善组和未显著改善组的差异有统计学意义(P<0.05),右向左分流和侧支循环建立是与“显著改善”相关的独立预测因素(P<0.05)。结论血管超声检查可以获得有价值的影像学和血流动力学信息,便于床旁操作、动态观察,可广泛应用于IVT诊疗过程中。Objective To analyze the relationship between the characteristics of vascular ultrasound and the effect of IVT in ischemic stroke.Methods First TCD examination was performed when IVT and vascular ultrasound examination was performed within 24 hours after IVT.The relationship between the characteristics of vascular ultrasound and"remarkable improvement"after intravenous thrombolysis was investigated.Results There were significant differences in collateral circulation,atrial fibrillation,ulcerative plaques and RLS between the two groups(P<0.05).Right-to-left shunt and collateral circulation were independent predictors of remarkable improvement after intravenous thrombolysis(P<0.05).Conclusion The vascular ultrasound examination can obtain valuable imaging and hemodynamic information,which is convenient for bedside operation and dynamic observation,and can be widely used in the diagnosis and treatment of IVT.
