摘　　要：目的探讨右美托嘧啶预处理对听神经瘤切除术患者术中脑氧代谢及血清NSE蛋白的影响。方法选取郑州大学第二附属医院择期行听神经瘤切除术患者40例,随机分为对照组(C组)、右美托嘧啶预处理组(D组),每组20例,C组采用丙泊酚联合瑞芬太尼麻醉方案,D组在C组基础上采用右美托嘧啶0.5μg/kg预处理,后以0.2~0.4μg·kg-1·h-1维持,记录麻醉后(T1)、切皮时(T2)、切肿瘤时(T3)、关颅时(T4)各组动脉、静脉血气、ELISA法检测NSE。结果T4时SjvO2组间比较差异有统计学意义(P<0.05),NSE蛋白组间比较差异有统计学意义(P<0.05)。结论右美托嘧啶预处理有助于改善听神经瘤切除术患者术中脑氧合状况,降低NSE蛋白的表达,减轻术中脑损伤的发生。Objective To investigate the effects of dexmedetomidine preconditioning on intracerebral oxygen metabolism and serum NSE protein in patients undergoing acoustic neuroma resection.Methods 40 patients undergoing elective acoustic neuroma resection were randomly divided into two groups,control group(group C)and dexmedetomidine preconditioning group(group D).Group C were treated with propofol combined with remifentanil anesthesia,while group D were pretreated with dexmedetomidine at a concentration of 0.5μg/kg on the basis of group C and the concentration was maintained at 0.2-0.4μg/kg·h.T1(after anesthesia induction),T2(cutting skin),T3(removing the tumor),T4(closing cranial),the arteries,venous blood gas were respectively recorded and the NSE were also tested with the ELISA method.Results The statistics of SjvO2 and NSE between the two groups at the stage of T4 is significant(P<0.05).Conclusion Dexmedetomidine pretreatment can optimize the intracerebral oxygenation statusin patients with acoustic neuroma resection,decreasing the expression of NSE protein and reducing the occurrence of intraoperative brain injury.
