摘 要:目的探索BRAF^V600E基因状态以及miRNA-221(miR-221)、miRNA-222(miR-222)在甲状腺乳头状癌(PTC)中的表达情况与其发病和预后的相关性。方法所有标本来源于2016年2月~2018年12月在新疆医科大学第一附属医院甲状腺进行外科手术的PTC患者,包括40例甲状腺乳头状癌组织和12例癌旁正常甲状腺组织。应用聚合酶链式反应(PCR)技术进行BRAF^V600E基因状态检测及通过应用实时荧光定量聚合酶链反应(RT-PCR)方法检测miR-221、miR-222的表达水平。结果⑴BRAF^V600E基因突变在PTC病灶中的阳性突变率为67.5%,正常甲状腺组织中未发现有表达,且与患者的淋巴结转移和包外膜侵犯情况显著相关(P<0.05)。⑵miR-221、miR-222在PTC患者中的相对表达水平显著高于正常对照组(P<0.05)。miR-221和miR-222的表达表现出与患者包外膜侵犯情况的相关性(P<0.05),仅miR-221的表达在有无淋巴结转移情况中表现出显著性差异(P<0.05)。⑶miR-221、miR-222在PTC患者中的表达与BRAFV600E基因突变成显著正相关(P<0.05)。结论BRAF^V600E突变和miR-221、miR-222的表达水平可作为判断PTC发病和预后情况的新指标。Objective To study the BRAF^V600E mutation and the expression of miRNA-221(miR-221),miRNA-222(miR-222),in papillary thyroid carcinoma and their roles in the carcinogenesis of PTC.Methods All specimens were obtained at First Affiliated Hospital with Xinjiang Medical university by core needle biopsy and open biopsy from February and 2016 to December 2018.There are 40 cases of PTC and 12 cases of adjacent normal thyoid tissues.The BRAF^V600E mutation was detected by PCR,DNA sequencing assays and immunohistochemistry(ICH).RT-PCR was used to detect the expression of miR-221,miR222.Results(1)The BRAF^V600E mutation was found in PTCs with the ratio of 67.5% and not detected in normal thyroid tissue,which was related to lymph node metastasis and capsular invasion.(2)The expression level of miR-221,miR-222 were significantly higher in PTC compared with adjacent normal thyroid tissue(P<0.05).The expression of miR-221 and miR-222 had a significant correlation with capsular invasion(P<0.05).OnlymiR-221 expression presented significant difference,lymph node metastasis(P<0.05).(3)The BRAF^V600E mutation had a significant correlation with the expression of three miRNAs(P<0.05).Conclusions The BRAF^V600E mutation and the expression of miR-221,miR-222 are useful prognostic and pathogenic biomarker of PTC.
