干扰素凝胶结合口服中药治疗宫颈人乳头瘤病毒感染患者的有效性和安全性研究

摘　　要：目的探讨干扰素α-2b凝胶结合口服中药治疗宫颈人乳头瘤病毒(HPV)感染患者的有效性和安全性。方法从2017年9月-2019年6月期间杭州市中医院收治的高危型HPV(hr-HPV)感染(中医辨证为湿热毒邪内蕴兼肝肾亏虚型)患者中随机选取134例,抽签均分为对照组和观察组,每组67例。所有患者入组后均采用重组人干扰素α-2b凝胶治疗,阴道给药1支/次,隔日用药,连续使用10次为1个疗程,共治疗3个疗程;对照组不予其他干预,观察组在此基础上结合中医辨证拟定方口服中药治疗,每日1剂,早晚分2次服用,30 d为1个疗程,治疗3个疗程。对比分析两组用药前后hr-HPV DNA检测结果、中医证候评分及相关指标变化,统计HPV转阴率、治疗有效率,观察不良反应情况。结果治疗前,两组hr-HPV DNA病毒载量、中医症候评分以及白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、IL-2/IL-4水平差异均无明显统计学意义(P>0.05);疗程结束后,观察组hr-HPV DNA病毒载量显著降低,中医症候评分减少,阴道冲洗液中细胞因子IL-2/IL-4增加,与治疗前、对照组比较差异有统计学意义(P<0.05);观察组HPV转阴率、治疗有效率均高于对照组,组间差异有统计学意义(P<0.05);治疗期间两组患者均未表现出不良反应。结论重组人干扰素(IFN)α-2b凝胶结合口服中药治疗宫颈HPV病毒感染(湿热毒邪内蕴兼肝肾亏虚型)效果确切,可明显改善患者临床症状、提高HPV转阴率,促进宫颈局部免疫平衡,具有良好安全性。Objective To explore the effectiveness and safety of interferon gel combined with oral Chinese medicine in treatment of patients with human papillomavirus(HPV)infection.Methods From September 2017 to June 2019,134 patients with high-risk HPV infection(traditional Chinese medicine syndrome differentiation:dampness-heat,noxious blood stasis,the deficiency of liver and kidney)treated in Zhejiang Chinese Medical University were randomly selected and divided into control group and observation group,67 cases in each group.All the patients were treated by recombinant human interferonα-2b gel of vaginal application,once a day,one course of treatment included 10 times,all the patients were treated for 3 courses of treatment,based on routine therapy,the patients in observation group were treated by oral Chinese medicine,daily one agent,twice a day,one course of treatment included 30 days,all the patients were treated for 3 courses of treatment.The changes of high-risk HPV DNA detection results,traditional Chinese medicine syndrome scores,and related indexes before and after treatment in the two groups were compared and analyzed.The negative conversion rates and effective rates of treatment in the two groups were calculated.The adverse reactions were observed.Results Before treatment,there was no statistically significant difference in high-risk HPV DNA load,traditional Chinese medicine syndrome score,and levels of interleukin-2(IL-2),IL-4,IL-2/IL-4 between the two groups(P>0.05).After treatment,high-risk HPV DNA load in observation group decreased significantly,traditional Chinese medicine syndrome score decreased significantly,IL-2/IL-4 ratio in vaginal washing fluid increased significantly,compared before treatment in observation group and control group,there were statistically significant differences(P<0.05).The negative conversion rate and effective rate of treatment in observation group were statistically significantly higher than those in control group(P<0.05).In the course of treatment,no adverse reaction was obser
